Objective: To evaluate the postoperative recurrence rate、mortality at the postoperative truncatedtime and the outcome of pregnancy after fertility-sparing surgery of the early epithelial ovariancancer (EOC)。Method:Using the datebase systerms include PUBMED、Cochrane Library、EMBAS、MEDLINE、(CNKI)、Wanfang and CBM。At the same time, Jilin University library were usedfor collect the related literatures。The retrieval period is not restricted and the language is set toChinese and English, keywords set to "epithelial ovarian cancer","epithelial ovariancarcinoma","fertility preservation","recurrence","pregnacy""fertility-sparing surgery","conservative Surgery","epithelial ovarian cancer","preserve fertility","recurrence","pregnancy" were initial literature search and retrieval。For all the preliminary acquisition ofliterature by reading the title,abstract and full text gradually after evaluation, extraction,stripping/reference,verification,bring into all conform to the standards of the prospective case-controlstudies,retrospective case-control studies and case series studies。All the literatures which accordto the standard used finally the RevMan5.3software to analysize。Results:Overall,13studies met the inclusion,including6articles which about the early ovarianepithelial carcinoma patients who were treated by fertility preserving operation or radicaloperation on the postoperative recurrence rate,mortality and postoperative censored time relatedto the pregnancy outcomes of case-control studies and case series of7articles on the reproductivefunction of hand after pregnancy outcomes for patients with preserved early ovarian epithelialcarcinoma。From these studies,total927patients had the early epithelial ovarian cancer。The6case control studies included239cases of reproductive function preserved early operation ofepithelial ovarian cancer and201cases underwent radical operation of early epithelial ovariancancer patients,487cases were reserved reproductive function of the early operation of epithelialovarian cancer in7part series case study。6case-control studies were retrospective studies。Metaanalysis showed that the early ovarian epithelial carcinoma patients with preservation ofreproductive function and operation in patients after radical operation of the overall recurrence rate (OR=1.25,95%CI=[0.71,2.19],P=0.44) and truncated time mortality (OR=1.49,95%CI=[0.76,2.92] and P=0.25) were slightly increased, no significant difference in therecurrence rate, two groups of FIGO phase IA for preservation of reproductive function epithelialovarian cancer patients after operation (OR=1.01,95%CI=[0.49,2.10] and P=0.97) and truncatedtime mortality (OR=1.04,95%CI=[0.45,2.39] and P=0.92) no significant difference.。13articleswere recorded for fertility preservation operation postoperative pregnancy outcomes,including atotal of6literatures detailed record of trying to conceive,successful pregnancy, full-terminfants,the number of premature abortion and ectopic pregnancy and records of congenitalmalformation cases after delivery of the fetus,calculate the pregnancy rate was62.1%, all thefetuses were no congenital malformations, good pregnancy outcome。Conclusion:For the early epithelial ovarian cancer patients who preserved the fertilityfunction, although slightly increased the postoperative recurrence rate and mortality censoredtime, but without statistical significance,。 For stage IA epithelial ovarian cancer patients for thepreservation of reproductive function operation did not increase the postoperative recurrence rateand mortality censored time, conservative operation and postoperative saved patients with normalreproductive function and gynecological endocrine function and postoperative pregnancyoutcome is good, also can improve the quality of life of patients and the daily life and personalconfidence.
