Part ⅠEvaluation of Left Atrial function in healthy people with Doppler Echocardiography and Volume Tracking MethodPurpose :To investigate the change of spectral Doppler echocardiography and left atrial volume in healthy people.Methods:125 healthy people were elected. The variable of pulmonary venous flow pattern(PVFP)and mitral valve flow pattern(MVFP) was record. peak velocity(E, A), deceleration time(DT),peak velocity(S, D, AR). The left atrial endocardial surface was traced by left atrial tracking.to obtaine curve of left atrial volume index-time and curve of left atrial volume diversification rate,record LA maximal volume(Vmax),LA minimal volume(Vmin),LA presystolic volume(V pre),and calculate LA active emptying volume(AEF),LA passive emptying fraction(PEF),systolic left atrial fill rate(d V/dts),the early diastolic left atrial fill rate,(d V/dt E),the late diastolic left atrial fill rate(d V/dt A), all measurements were corrected to body surface area.Results :(1)MVFP variables of A were increased and E,E/ A were decreased with age(p<0.05).PVFP variables of S, S/ D, ARwere increased and D, were decreased with age(p < 0.05).(2)Vmax、Vmin、 Vpre、 LAEF、AEF、d V/dts、d V/dt A were increased and PEF、d V/dt E were decreased with age(p<0.05).(3)E and PEF,D and PEF,A and AEF,AR and AEF,S and d V/dts related well with each other(p<0.05)Conclusion: There is increased LA volume with change of age and imporve atrial pump ability and reduce of left atrial conduit function.Left Atrial Volume Tracking Method combining with pulmonary venous flow pattern and mitral valve flow pattern can be comprehensive evaluate atrial function.. There is Intrinsic relationship between PVFP and MVFP and LAVT.Part ⅡEvaluation of left atrial function by three-dimensional echocardiography in patients with mitral annular calcificationPurpose : To study the relation between MAC with LA volume and function.Methods:This study included 41 patients with MAC and 30 healthy controls,The patients were divided into two groups according to the severity of MAC,the patients subdivided into two subgroups.G1:patients with mild MAC,G2:patients with moderate to severe MAC.In addition to conventional 2D echocardiographic measurements, RT3 DE was performed to assess LA volumes and function. LA maximum volume(Vmax),LA minimum volume(Vmin): before atrial contraction volume(Vpre),was record. From the three volumes, the following measurements were selected as indices of LA function : LA total stroke volume(TSV); LA total emptying fraction(TEF); LA active stroke volume(ASV);LA active emptying fraction(AEF); LA passive stroke volume(PSV);LA passive emptying fraction(PEF);LA expansion index(EI).Results :(1)RT3DE demonstrated Vmax,Vmin,Vpre were significantly higher and TSV,TSV,ASV,EI,LAEF, AEF were found to be significantly lower for MAC patients compared with controls(2)Vmax,Vmin,Vpre,were found to be significantly higher and TSV,TSV,ASV,EI,LAEF, AEF were found to be significantly lower in G2 compared with G1,There was no significant difference between groups regarding PSV and PEF.Conclusion: Patients with MAC were found to have increased LA volume and impaired atrial compliance and the atrial pump ability. There was no significant difference of left atrial conduit function. Evaluation of MAC patients by using RT3 DE atrial volume analysis may facilitate recognition of subtle myocardial alterations related with MAC.
