| Purpose:Analysis the curative affection of Dachengqi Decoction on patients with sepsis associated ARDS, and analysis the serum concentration changes in interleukin 8(IL- 8), angiotensin- 2(Ang- 2), soluble receptors advanced glycosylation end products(s RAGE), to explore the relationship between those bio-markers and ARDS, and discussion the situation that with the treatment of Da chengqi Decoction, patients with sepsis associated lung injury, were getting better and provide the scientific essence for the basic theory of traditional Chinese medicine:lung and large intestine are pathophysiologly associated‖.Materials and Methods:A total of 44 patients with sepsis associated lung injury who were admitted consecutively between 2013 March and 2015 March were recruited in No.1 Hospital of Dalian Medical University,emergency ICU. they were randomized into the western medicine group(n22,routine treatment of Western medicine plus placebo,placebo is warm sail water)and the integrated Chinese and Western medicine group(n=22, routine treatment of Western medicine plus, Da cheng qi Decoction oral/tube injection/enema,2times/d,continuous administration for 5-7days). Record patients’ age, sex, disease and the score of APACHE Ⅱ/ ALI, and record the data when selected(d1), 3 days(d3) after treatment, and 5 days(d5) after treatment, and7 days(d7) after treatment, record the APACHE II score, LIS score, Ang- 2, IL- 8, s RAGE level changes; Defecation, exhaust time of the patients were observed, and sum up the 28-day mortality statistics.Results: The basic line of both group, the level of serum Ang- 2, IL- 8 and s RAGE, the score of APACHE Ⅱ/ ALI were no statistically significant difference(P > 0.05), after treatment, Ang- 2, IL- 8 and s RAGE level were decreased in both group, and the overall trend were statistically significant different. The mean estimate the marginal figure showed that serum Ang- 2, IL- 8 and s RAGE in the treatment group were fall in D3,and in D5, fall more obvious; APACHE II score in both groups, there was no statistically significant difference LIS score overall trend, but each time point average decline is obvious; The treatment group patients with intestinal function recovery time was significantly shortened.Conclusions:With the treatment of Dachengqi Decoction, serum Ang- 2, IL- 8 and s RAGE were decreased significantly in patients with sepsis associated ARDS, and the symptoms such as abdominal distension constipation were improved, the curative affection is better than pure western medicine treatment.The lower levels of Ang- 2, s RAGE prompt the recovery of alveolar endothelial function, the improvement of the alveolar capillary endothelial barrier permeability; The lower levels of IL – 8 may enlighten the better outcome of sepsis related ARDS. Contrast the two group, with the treatment of Dachengqi Decoction, patients may have a better prognosis and clinical symptoms are improved. |
