Factors Analysis On The Health Services Need And Utilization Of Elderly In Zhejiang And Gansu Province

Objectives: Through the analysis of the data of Zhejiang and Gansu province in the year of 2008 and 2012, which reflect the dynamic change of health service need and utilization. Seek to the cause of block the elder’ use of health utilization, affecting factors, analysis existing problems, and provide the basis for relevant health authorities to make decisions.Methods: By analyzing the data of China Health and Retirement Longitudinal Study(CHARLS) in 2008 and 2012, using Probability Proportional to Size Sampling( PPS), determine the respondents by sampling at county level, village level, household level and choose candidate. Using survey data to do the single factor analysis and multifactor regression analysis, single factor classification data by chi-square test, multi-factor analysis using logistic regression method to analyze the influencing factors. ?=0.05 is the test level.Results: Weed out incomplete data, a total of 2008 survey over 60 elderly population of 1007 people, including 549 from Zhejiang province, 338 households and 458 from Gansu province, 404 households; a total of 2012 survey over 60 elderly population of 1334 people, including 721 from Zhejiang province, 495 households and 613 from Gansu province, 425 households. The two provinces on the elderly health services needs is as follows: the elderly in 2008 four-week prevalence was 32.77%, in 2012 four-week prevalence was 29.65%. The chronic disease prevalence rate was 64.85% in 2008 and 78.86% in 2012. The two provincial health services utilization among the elderly is: the 2008 and 2012 survey of self treatment rate of elderly in Zhejiang province was 48.09% and 18.78%. And the self treatment rate of elderly in Gansu province was 50.92% and 72.59%. Total four-week visiting in 2008 was 22.24%, in 2012 was 23.01%. Total hospitalization in a year in 2008 was 8.74%, in 2012 was 12.82%. On the choice of clinics, the old people in zhejiang province preferred general hospital, old people in gansu province prefer private clinics, and village clinic,among the hospital site, general hospital is the main, choose general hospital in zhejiang province is higher than the proportion of gansu province. Health expenditure levels, whether suffering from chronic diseases, self-reported health status is the common factors that affected the elderly utilize health services both in Zhejiang and Gansu province.Suggestions: 1. Improve the level of economic development of western region in China. 2. Increasing investment, strengthen the elderly health education, improve the medical network at all levels to improve the elderly health. 3. Strengthen the prevention and treatment of chronic diseases, control the development of the elderly chronic diseases. 4. Focus on specific people’s health. 5. The insurance of New Rural Cooperative Medical system(NRCMS) should be continued to carry out. Raise the level of reimbursement on the basis of expanded coverage of NRCMS.
