Objective: The study is to analyse the Knee joint’s function, stability, Changes of biomechanical parameters and clinical significance when walking on different slope after ACL injury, by Opti-Knee Three-dimensional movement gait system. Methods: The Opti-Knee knee motion gait analysis system monitoring ipsilateral and contralateral knee activity for the ACL injury patients in the 0 degree and 15 degree flat when walking on the uphill and downhill, Recording displacement distance, internal rotation angle and varus angle between bilateral knee joint and anterior tibial, Analyzing and comparing of bilateral knee joint stability after ACL injury.Results:(1)When patients with ACL injury walking in different states, the forward displacement, internal rotation angle and varus angle of the injured side was significantly greater than the uninjured side(P<0.05).(2)When patients with ACL injury walking at 15 degrees up and down slope, the forward displacement, internal rotation angle and varus angle of the injured side was significantly greater than the uninjured side(P<0.05).(3)No significant difference of the forward displacement, internal rotation angle and varus angle of uninjured side between walking at 15 degree downhill and walking at 15 degree uphill for ACL injury patients.(P>0.05).(4)When walking in 15 degree downhill, the forward displacement, internal rotation angle and varus angle of the injured side was significantly greater than walking in 15 degrees uphill(P<0.05).(5)When ACL injury patients walking in 15 degree downhill and 15 degree uphill, sensitivity of the uninjured side had no significant difference(p>0.05). When ACL injury patients walking in 15 degree downhill, the knee of the injured side is more sensitive than walking in 15 degree uphill. Conclusions: ACL is crucial for the function of knee joint, ACL injury seriously affects the function and stability of the knee, different inclination have different affection of the ACL injured knee.(1) Uninjured side an ACL injury of knee joint on the downhill movement when walking than the ground stability obviously, but when the contralateral knee joint is on a downhill movement has no obvious difference.(2) Affected side an ACL injury of knee joint on the downhill movement when walking than the ground stability obviously.(3)The knee of injured side the forward displacement, internal rotation angle and varus angle is much sensitive when walking at 15 degree downhill than walking at 15 degree uphill. This can also draw a conclusion that the stability of the knee after ACL injury getting weaker when walking at up and down slop, especially weak when walking at downhill, meanwhile may further damage the ACL injury, eventually lead to degeneration of knee joint or irreversible repair. Three dimensional knee movement gait analysis system for quantitative evaluation of knee joint function after injury by ACL, contribute to a more accurate grasp of ACL injury of knee joint kinematics changes, so as to provide basis for clinical diagnosis and treatment.
