The Expression And Significance Of MMP-3 In Middle Ear Cholesteatoma

ObjectiveTo detect the expression of the MMP-3 (Matrix metallo proteinase-3) in case of cholesteatoma, in comparison of the external canal’s normal skins and the granulation tissue in the middle ear while chronic purulent otitis of the middle ear, and classify the different degree of the temporal bone destruction according to the pathological changments in the cholesteatoma epithelium tissues of the middle ear. To define the position of the MMP3 for the bone destruction in case of cholesteatoma.MethodsTo choose 30 patients’cholesteatoma epitheliums removed from the operation with different ages and genders whose diagnosis is middle ear cholesteatoma with bone destr-uction and absorption as experimental group,collect 30cases of the non chronic middle ear otitis and cholesteatoma patients’normal external canal skins and 25 cases of the granulation tissues from the chronic suppurative otitis of the middle ear for the control group, marked group A and B. Mark Ⅰ, Ⅱ,Ⅲ and Ⅳ degree according to the temporal bone destruction degree in case of middle ear cholesteatoma in thr experimental group,use HE coloration and Max Vision TM immunohistochemistry method to detect the expression of MMP3 in experimental and control groups, use computer digital pathological image analysis system to observe and count the positive expressions. Using SPSS statistical analysis software package to analyse the data, P<0.05 was considered statistically significant. Analyse the relation of MMP3 expression and bone destruction of the middle ear chlesteatoma.ResultsThe positive expressions of MMP3 in the cholesteatoma epithelium exist in all the epitheliums,located in the brun granulesof the cytoplasms,the expression of MMP3 in cholesteatoma epithelium(Positive cell/grey value(Mean+SD):31.39 ±8.37,198.83 ± 19.32 P<0.05) is higher than all the nomal external canal’s skins(1.93 ± 1.46,239.45± 0.88 P< 0.05) and the group of middle ear chronic suppurative otitis(24.99±5.41,198.83 ±19.32 p<0.05).the expression of the mmp3 in epithelium of cholesteatoma 30cases and temporal bone’s bonedestruction from I degree to IVdegree.Use positive cell and grey value as a standard of comparison, At the first,using one factor analysis of variance and least significant difference t test during the four groups to contrasting them. There was statistical significance between the groups, (P<0.05).There was statistical significance between the groups (P<0.05).At last using spearman’s rank correletion test,there was strong correlations exist between the expression of the MMP3 in epithelium of cholesteatoma and temporal bone’s bonedestruction when confidence level was 0.01.ConclusionsMMP3 has a higher expression in case of cholesteatoma epithelium than that in the normal skin and middle ear chronic suppurative otitis,the expression in case of middle ear chronic suppurative otitis, the expression in the case of middle ear chronic supperative otitis is higher than that in the normal external conduit skin,MMP3 take part in the evaluation of bone destruction and absorption of the middle ear cholesteatoma.
