Objective: through to the inferior vena cava filter implantation combined catheter thrombolysis directly the effect of the treatment of acute lower extremity deep vein thrombosis and safety evaluation, provide the basis for clinical directly to carry out the catheter thrombolysis and treatment experience. Methods: the medical school first affiliated hospital of south anhui vascular surgery in January 2012 to January 2014 admitted during the period of 100 cases of acute lower extremity deep vein thrombosis in patients with clinical data were retrospectively analyzed. According to different therapy and thrombolytic ways can be divided into vein system thrombolysis and direct catheter thrombolysis group, two groups of the 50 patients, all patients with catheter thrombolysis group directly through thrombolytic infusion catheter thrombolysis, before thrombolysis were implanted into the inferior vena cava filter first. System thrombolysis group of 50 cases with limb peripheral venous thrombolysis, not conventional implantation of inferior vena cava filter. Through observation and follow-up after discharge to lower limb week radius change, venous patency rating, venous patency rate, and according to the Villalta grading standard incidence of thrombosis syndrome after judgment to evaluate therapeutic effect and complications such as bleeding, pulmonary embolism, evaluating its security, compare the near future, the clinical curative effect and safety of the two groups. Results: Catheter thrombolysis in the group without the occurrence of cases of pulmonary embolism, and there are 6 cases occurred in system thrombolysis group of symptomatic pulmonary embolism, which compared with statistical difference(P < 0.05); Catheter thrombolysis ham and crus weeks path difference before and after the treatment group is significantly higher than system thrombolysis group(P < 0.05). Venous patency rate before and after catheter thrombolysis treatment group is significantly higher than system thrombolysis group(P < 0.05), are statistically significant. Complications during thrombolysis and catheter thrombolysis group on the basis of Villalta ratings, the incidence of PTS significantly below system thrombolysis group(P < 0.05); Conclusion: inferior vena cava filter implantation can effectively prevent the occurrence of pulmonary embolism; Catheter thrombolysis directly compared with intravenous systemic thrombolysis, catheter thrombolysis treatment with good results, directly thrombus dissolving speed, in the process of thrombus thrombolysis complications and the incidence of venous thrombosis syndrome after the advantages of low. Catheter thrombolysis combined directly inferior vena cava filter implantation is effective for acute lower extremity deep vein thrombosis, safe treatment method, is worthy of physicians in clinical widely.
