| Background:Diffuse interstitial lung disease (DILD, ILD) is a large group of heterogeneity of diseases which are characterized by nonspecific alveolar inflammation associated with interstitial fibrosis,which is diffuse unit as alveolar, non tumor, non-infectious. Idiopathic pulmonary fibrosis (IPF) as one of the most representative type in the DILD. Those course being slow progress, DILD is gradualiy loseing alveolar-capillary function unit, and finally arrive in the development of diffuse pulmonary fibrosis and honeycombing lung, eventually die because of chronic cor pulmonale, respiratory failure. This disease is a deadly disease, the end of last century the morbidity of this disease is very low, morbidity of this disease is begin to go up the turn of the century. the disease in some big hospital outpatient and inpatient significant proportion of climbing. At present the treatment of the disease is still a worldwide problem, and lack of effective drugs, now advocates the use of acetylcysteine effervescent tablets, the capsule bailing, but these efficacy is limited. Meanwhile the curative effect of expensive pirfendidone just being at the beginning of 2014 on the market in our country need further evaluation. DILD has seriously threatened the life and health of many people in our country, the task of prevention and cure of the disease is imminent. DILD included in more than 200 diseases,which have their own characteristics by each type of clinical manifestation, laboratory and pathology change, but they have some common clinical, pathologic physiology and chest iconography characteristics. Based on the traditional Chinese medicine(TCM), various types share a common pathogenesis of DILD directed by the theory of "yi bing tong zhi". this study only represented by IPF on TCM syndrome differentiation and treatment. TCM has a history of long standing and well established in the disease prevention and control, also has a unique advantage and curative effect, preventing and treating difficult disease from the perspective of Chinese medicine can provide broad field of vision for the treatment of DILD. Tian Zhengjian professor of Traditional Chinese Medicine hospital in hubei province respiration medicine argues to prevent and control DILD from the perspective of "yiqi huoxue tongluo"Objective:The prescription by applying the method of "yiqi huoxue tongluo" treatment of idiopathic pulmonary fibrosis (IPF). To observe the clinical curative effect of traditional Chinese medicine treatment of the disease and security, the study explore a preventive and cured strategy diffuse interstitial lung disease (DILD) applying traditional Chinese medicine,which also provide evidence-based medicine for Traditional Chinese Medicine syndrome differentiation about IPFMethod:The outpatient and the inpatient in Traditional Chinese Medicine hospital in hubei province respiration medicine during April 2009-December 2014, which is including in 40 patients with IPF, randomly divided into experimental group and control group,20 cases in each group. Control group be give family oxygen cure, glucocorticoid, anti-infection, acetylcysteine effervescent tablets and other support therapy; The treatment group in the control group on the basis of the comprehensive treatment plus for reasons based on traditional Chinese medicine on method of "yiqi huoxue tongluo". Two groups of patients before treatment level of gender, age, course of disease, difficulty breathing level,6 minutes walk test (6MMT), pulmonary function, SGRQ scores and 1 year data such as the average number of exacerbations and after treatment pass statistics test, which was no significant difference, and was comparable. Observation time is 2 years. Were observed before and after treatment of patients with dyspnea, acropachia, cyanosis of lips, lung Velcro rale, pulmonary function of VC MAX, chest CT scan, PaO2 of arterial blood gas analysis,6 minutes walk test (6MWT), average hospitalization times within 1 year, st. George’s questionnaire (SGRQ), and other indicators of change. All data are use SPSS19.0 statistics. All measurement data using as "mean+standard deviation" (x±s), between the groups choose t test to compare; Count data using as X2 test comparison, the Ridit analysis being used in ranked data. P<0.05 show that the difference was statistically significant. Result:1. Two groups of clinical curative effect comparison:treatment group total effectiveness 80.00%, control group total effective rate 55.00%, two groups of comparison, significant differences (P < 0.05). Results indicate that curative effect was obvious that the Traditional Chinese Medicine for idiopathic pulmonary fibrosis disease early, relatively light, however curative effect is common when the disease is in heavier and later. It is prompt treatment of idiopathic pulmonary fibrosis should be early2. Within the group before and after treatment symptoms and signs, auxiliary test related indexes such as integral comparison: treatment group total score after treatment than before treatment significantly decreased, significant differences (P<0.05). Compar-ing the two groups after treatment, differences among the treatment group in all aspects except lung Velcro rale were significant (P<0.05), which suggests that the treatment group was better than control group in improving patients’ quality of life.indeeding exercise endurance, improving lung function, reducing the risk of exacerbations,and so on.Conclusion:Through the observation of clinical curative effect of Trditional Chinese Medicine, explain to Trditional Chinese Medicine treatment of Syndrome differentiation about "yiqi huoxue tongluo" in the prevention and control of IPF is indeed better than pure Western Medicine therapy. Advocates of early treatment, prevention before being disease,control which lead to disease;promote the combination of Trditional Chinese Medicine and Western Medicine and other comprehensive treatment. |
