Objective 1. To analysis the changes of Th1, Th2, Tfh cells and the expression of their transcription factor T-bet m RNA, GATA3 m RNA, Bcl-6 m RNA in peripheral blood of HIV/AIDS patients;2. A preliminary study on the relationship between Tfh cells and the progression of HIV/AIDS by analysis the correlation of Tfh and CD4+T cells.Methods This experiment chose 61 patients who were confirmed by HIV antibody test positive, during April 2014 to November from AIDS Research Institute in the First Affiliated Hospital, Henan College of Traditional Chinese Medicine. According to the amount of CD4+T cells, the patients with HIV/AIDS were divided into A(CD4≤200 cells/μl), B(200<CD4<500 cells/μl) and C(CD4≥500 cells/μl) three groups. At the same time, we chose 20 persons from Physical examination center as healthy control group. Percentages of lymphocyte subsets(CD3+T cells, CD3+CD4+T cells, CD3+CD8+T cells, CD3-CD16+CD56+ cells, CD3-CD19+ cells), Th1(CD4+IFN-γ+), Th2(CD4+IL4+) and Tfh(CD4+CXCR5+ICOS+) cells were investigated by Flow cytometry(FCM). The expression of Bcl-6 m RNA, T-bet m RNA and GATA3 m RNA levels were detected by reverse transcription-polymerase chain reaction(RT-PCR). To analysis the relationship between Tfh cells and CD4+T cells, and Tfh cells association with Bcl-6 m RNA and B cells in HIV/AIDS patients.Results 1. To analysis the changes of lymphocyte subtypes: percentage of CD3+CD4+T cells of HIV/AIDS patients were significantly lower than the healthy control group(P<0.05), percentage of CD3+CD8+T cells of HIV/AIDS patients were significantly higher than the healthy control group( P<0.05).There was no significantly different of CD3+T cells, B(CD3-CD19+) cells and NK(CD3-CD16+CD56+) cells ratios between the four groups(P>0.05).2. To analysis the changes of Th1/Th2/Tfh cells and their corresponding transcription facor: the percentage of Th1 cells of group A(13.56±8.47)was higher than the rest of three groups(P<0.05). And there was no significantly different of Th1 cells between the rest of three groups(P>0.05). The expression of T-bet m RNA, which was the transcription factor of Th1, of group A( 11.06±4.87),B( 10.21±3.83) and C(7.86±3.95)was increased than the healthy control group(P<0.05). Comparing differences between Th2 cells in four groups had no statistical significance. GATA3 was Th2 cells’ transcription factor. The expression of GATA3 m RNA of group A(1.95±0.45),B(2.49±0.91)and C(2.64±0.93)was increased than the healthy control group( P<0.05). Percentage of Tfh( CD4+CXCR5+ICOS+) cells of group A(5.27±2.34)was increased than group C(1.58±1.12)and healthy group(0.80±0.49)(P<0.05). The expression of Bcl-6 m RNA of group A(2.56±0.61),B(2.30±0.64)and C(2.02±0.49)was increased than the healthy control group(P<0.05).3. The correlation of different Tfh cells and Bcl-6 m RNA: correlation analysis showed that CD4+CXCR5+ICOS+ T cells was positive correlation with Bcl-6 m RNA(r=0.559,P=0.000). CD4+CXCR5+ T cells was positive correlation with Bcl-6 m RNA(r=0.275,P=0.032).4. The correlation of different Tfh cells and B cells: analyze of correlation between different Tfh cells and B cells revealed that there was no relationship between them(r=-0.102, P=0.363;r=-0.043, P=0.706).5. The correlation of different Tfh cells and CD4+T cells: correlation analysis showed that CD4+CXCR5+ICOS+ T cells was negative correlation with CD4+T cells(r=-0.749, P=0.000). CD4+CXCR5+ T cells was negative correlation with CD4+T cells(r=-0.731, P=0.000).Conclusion 1. The balance between Th1 and Th2 was broken in HIV/AIDS. The percentage of Tfh cells was abnormal and it was negative correlation with CD4+T cells, which indicated that Tfh cells might associate with the progression of disease. 2. Overexpression of Bcl-6 m RNA, T-bet m RNA and GATA3 m RNA revealed that Tfh,Th1 and Th2 cells involved in the pathogenesis of HIV/AIDS.
