Influence Of Different Contrast Media On Renal Function Of Patients Undergoing Coronary Angiography Or Percutaneous Coronary Intervention With Oral Hydration

[Objective] To compare the influence of low-osmolar contrast media (LOCM) with iso-osmolar contrast media (IOCM) on renal function of the patients undergoing coronary angiography (CAG) or percutaneous coronary intervention (PCI) with oral hydration.[Methods] A randomized controlled trial was performed in all patients of suspected coronary heart diseases who had undergone CAG or PCI at the Department of Cardiology of the College of Clinical Medicine of Yangzhou University from March 2014 to February 2015. All patients were given oral hydration and other routine treatments for CHD and randomly received one of the two different kinds of contrast media:low-osmolar or iso-osmolar contrast media. After eliminating a few patients who lacked necessary data and met the rule-out standards,167 patients were finally selected and divided into group ioversol (using LOCM—ioversol) 85 cases and group iodixanol (using IOCM—iodixanol) 82 cases, according to the type of contrast media which they were recieved. Clinical features, the volume of used contrast media, pre-and post-operative difference of serum creatinine (Scr), systatin C(Cys C) and estimated glomerular filtration rate (eGFR), the rate of contrast-induced nephropathy(CIN) of the two groups were obtained and analyzed.[Results] There were no statistical significant difference in the clinical features, the volume of used contrast media, the preoperative difference of Scr, Cys C, the eGFR between the LOCM group and the IOCM group(P>0.05). After operation there were Scr and Cys C increasing and eGFR decreasing with statistical significant difference(P<0.05) in both groups,but there were no statistical significant difference between the two groups (P>0.05). Amongl67 patients, CIN occurred in 7 patients (the incidence of CENT was 4.19%). CIN occurred in 4 patients in group ioversol, and 3 in group iodixanol, the CIN rate were higher in group ioversol than in group iodixanol (4.71% VS 3.66%), but there were no statistical significant difference between the two groups also(P>0.05).[Conclusion] 1.Low-osmolar and iso-osmolar contrast medium both have some injury on the renal function of patients undergone CAG or PCI.2. Low-osmolar and iso-osmolar contrast medium may have the samilar renal injury on patients with oral hydration who undergoed CAG or PCI.
