Epidural Postoperative Opioid Analgesic Effects On Gastrointestinal Motor Function In Patients Undergoing Cesarean Section

Objective : Research the impact of morphine,fentanyl,sufentanil during epidural analgesia on postoperative gastrointestinal motor function in patients with cesarean section, and watch for postoperative nausea and vomiting(PONV) and its analgesic effect.Methods:According to the postoperative epidural analgesia or not and uses of different analgesic drugs, eighty patients of cesarean section were divided into four groups(20 cases in each group): Group M: morphine 40ug?m L;Group S: sufentanil 0.4ug?m L;Group F: fentanyl 4ug?m L;Group C:patient refuses to use analgesia pump. Observed and recorded visual analogue scale(VAS) immediate postoperative, after operation 3h, 12 h, 24 h, 48 h, the incidence rate of postoperative nausea and vomiting, postoperative recovery time of bowel and anus exhaust the first time for each group of patients.Results:1.Gastrointestinal motor function:(1) Compared the other three groups, postoperative recovery time of bowel and anus exhaust the first time of group M was longer(P<0.05).(2) Gastrointestinal recovery time of motor function of group M, F, S was significantly longer than group C(P<0.05).(3) Gastrointestinal recovery time of motor function of group S was significantly faster than group M, F(P<0.05). 2.PONV:(1) The incidence rate of PONV of group M, F, S was significantly higher than group C(P<0.05).(2) Compared the rest groups, the incidence rate of PONV of group M was higher(P<0.05).(3) There were no significant difference in the incidence rate of PONV between group F and group S(P>0.05). 3.VAS:(1) Compared the other three groups, the VAS after operation 3h of group C was higher(P<0.05).(2) The VAS after operation 12 h of four groups were significantly higher than before(P<0.05).(3) The VAS after operation 12 h to 24 h of group C fluctuates in the 5-6 score.Conclusion:Compared to morphine and fentanyl, sufentanil can guarantee a good foundation on analgesia, the inhibition of sufentanil is weakest to gastrointestinal motor function for epidural analgesia after cesarean section.
