| PurposeAnalyze serological indexes, clinical characteristics, treatment and prognostic factors in hepatocellular carcinoma patients with bone metastases after liver transplantation.MethodWe collect15patients with bone metastases after liver transplantation in the first affiliated hospital of zhejiang university between January2010and March2015, including the serological indexes, clinical features, survival time.The possible factors influencing the prognosis of patients will be analysed through the comparison of serological indexes before and after bone metastasis, the survival time,the clinical characteristics and the treatment, combined with literature review analysis.ResultsThe15patients’ median survival time from the diagnosis of bone metastasis to death was5months and the median time from liver transplantation to the diagnosis of bone metastasis was8month.The overall6-and12-month survival rates were33%and 7%,respectively. The serum alpha fetoprotein (AFP) level before and after the bone metastasis did achieve statistical significance (P<0.05),while serum alkaline phosphatase (ALP) did not (P>0.05). The numbers of bone metastasis of axial skeleton, the extremities and both were8,2and5,respectively. Local pain is the main clinical manifestation in these patients.Palliative treatment can relieve the symptoms caused by bone metastasis, with a significant decline of Visual Analogue Scale (VAS)(P<0.05).ConclusionPatients with bone metastasis after liver transplantation have a poor prognosis. AFP level before and after the bone metastasis did achieve statistical significance, while ALP did not. Local pain is the main clinical manifestation in these patients. Palliative treatment can relieve the symptoms caused by bone metastasis. Status before liver transplantation and after the occurrence of bone metastases affects the prognosis of these patients on the basis of literature review and a15-cases analysis. |
