Objective: To discuss the clinical significance and prognostic value of 1p/19 q co-deletion and IDH1 gene mutation through detecting of 1p/19 q co-deletion and IDH1 gene mutation in malignant gliomas.Methods: From March 2012 to September 2014, eighty-four cases with malignant glioma confirmed by pathology were collected in our study. Anaplastic oligodendroglioma(WHO g rade Ⅲ) 6 cases, anaplastic oligoastrocytoma(WHO grade Ⅲ) 14 cases, anaplastic astrocytoma(WHO grade Ⅲ) 28 cases, glioblastoma(WHO grade Ⅳ) 36 cases. The co-deletion of chromosome 1p/19 q was investigated by PCR fragment analysis, IDH1 Gene Mutation was detected by PCR sequencing analysis. Collect clinical data, including age, gender, KPS score, pathological grade, p53 expression and Ki67 expression. The chi-square test was used to test the association between IDH1 mutation or and clinic pathologic features in malignant gliomas. Survival probabilities were estimated based on Kaplan-Meier method.Results:1. Among 84 cases, there were 24 cases(28.57%) with 1p/19 q co-deletion were found. 1p/19 q co-deletion was associated with age(P<0.05), pathological grade(P<0.05), p53 expression(P<0.05), Ki67 expression(P<0.05). Patients with 1p/19 q co-deletion showed a longer PFS than patients without 1p/19 q co-deletion; Meanwhile, patients with 1p/19 q co-deletion showed a longer OS than patients without 1p/19 q co-deletion.2. Among 84 cases, there were 10 cases(11.90%) with IDH1 gene mutation were found, all of which were R132 H type mutations. IDH1 gene mutation was associated with age(P<0.05), Ki67 expression(P<0.05). Patients with IDH1 gene mutation showed a longer PFS than patients without IDH1 gene mutation. Meanwhile, patients with IDH1 gene mutation showed a longer OS than patients without IDH1 gene mutation.3. Statistically significant correlation was found between 1p/19 q co-deletion and IDH1 gene mutation(γ =0.256,P=0.019)。Conclusion: Patients with 1p/19 q co-deletion or IDH1 gene mutation showed a longer PFS and OS than patients without 1p/19 q co-deletion and IDH1 gene mutation; 1p/19 q co-deletion and IDH1 gene mutation could be used as important indexes for prognosis. The treatment options were targeted to select under the guidance of 1p/19 q co-deletion and IDH1 gene mutation. It has important reference value for personalized treatment.
