Clinical Analysis Of Cystic Neoplasm Of Body And Tail Of Pancreas (32 Cases)

Objective:In the surgical treatment about cystic neoplasm of body and tail of pancreas,to compare the distal pancreatectomy with splenectomy with the spleen-preserving distal pancreatectomy about the effect of operation, to inquire into the feasibility and safety in the method of spleen-preserving distal pancreatectomy.Methods:Retrospectively analyzed the clinical date of 32 patients with cystic neoplasm of body and tail of pancreas from September 2007 to December 2014, in general surgery comment of the First Affiliated Hospital of Kunming Medical University,(among them,12 cases of serous cystic neoplasm,10 cases of mucinous cystic neoplasms,7 cases of intraductal papillary mucinous neoplasms,3 cases of solid-pseudopapillary neoplasm),including 15 cases of spleen-preserving distal pancreatectomy,in which 9 cases preservation of the spleen vessels method and 6 cases resection of the spleen vessel method,other 17 cases of distal pancreatectomy with splenectomy. By independent-sample T test and chi-square test to compare the difference between spleen-preserving distal pancreatectomy and distal pancreatectomy with splenectomy in intraoperative situation, length of postoperative hospital stay and platelet count.Results:the distal pancreatectomy with splenectomy (groupA) and the spleen-preserving distal pancreatectomy (group B) about the average operation time is no statistically significant; the average operation time about preservation of the spleen vessels method(group B1) was longer than the resection of the spleen vessel method(group B2), the difference is significant.the mean volume of operative bleeding in group groupB1 is much more than the group B2, the difference is significant.Conclusion:In the surgical treatment about cystic neoplasm of body and tail of pancreas with the method of spleen-preserving distal pancreatectomy is much more better than the distal pancreatectomy with splenectomy in hospital stay. We should preserve the spleen vessels when choosing the spleen-preserving distal pancreatectomy method,Although,the average duration of surgery and the mean volume of operative bleeding is inferior to the resection of the spleen vessel method group.But as a result of preserving splenetic vessels, which can reduce the the incidence rate of regional portal hypertension.
