Objective:To investigate the hemorrhagic cerebral infarction(HI) risk factors,clinical manifestations,imaging features,diagnosis and treatment and prognosis.In order to help clinical doctors improve recognition,make timely and correct diagnosis,early treatment and improving prognosis. Methods:By reviewing pertient literatures,Complete the clinical data of 852 patients with cerebral infarction in our hospital from December 2013 to December 2014 were collected,these data inclued the 40 cases of hemorrhagic cerebral infarction.To retrospectively analyzed 40 cases of clinical data of patients with hemorrhagic cerebral infarction and studied the risk factors,clinical manifestations,imaging features,treatment and prognosis.Results:The group of 40 cases of cerebral hemorrhage in patients accounted for the same period in 4.7% patients.For the top three of the risk factors were hypertension 27 cases(67.5%);20 patients(50%) with atrial fibrillation;large area cerebral infarction in 27 cases(67.5%);other risk factors were factors of diabetes mellitus,high concentration of low density lipoprotein,coronary heart disease,such as hypertension,which part of infarction,atrial fibrillation,large area cerebral infarction was the main cause of hemorrhagic cerebral infarction.Clinical and image manifestations of hemorrhagic cerebral infarction varied from patient to patient.The clinical manifestations of hemorrhagic cerebral infarction without specific,its common clinical manifestations was recognizant obstacle,limb movement disorder,language barriers,eyeball movement disorder,hemianopsia,abnormal pupil,dizziness headache,diplopia,ataxia,paralysis,face and tongue sensory disturbance.In the group,the top three of clinical manifestation were disturbance of consciousness in 10 cases(22.5%), one side limb movement disorder in 20 cases(50%),12(30%) patients with language disorder.The group of 40 cases of cerebral hemorrhage patients in cortex group 36 cases, non cortical group 4 cases,CT examination revealed hemorrhagic infarction lesions in 38 cases(95%),MRI examination revealed hemorrhagic infarction lesions in 40 cases(100%),including 1 cases of subacute hemorrhagic infarction,MRI provides diagnosis.This research object belongs to the 40 cases of HI and 11 cases of type I,type II in 13 cases,type III in 10 cases,type IV in 6 cases.Before treatment,in all samples with hemorrhagic cerebral infarction of NIHSS score than or equal to eight points were 17 cases(42.5%),NIHSS score less than eight points were 23 cases(57.5%),The main NHISS scores were 1 point(4cases,10%),2 points(6 cases,15%),3 points(6 cases, 15%).HI has poor prognosis,but if the diagnosis and treatment in time,could improve the survival rate and reduce the mortality rate,in all of hemorrhagic cerebral infarction,32 cases(80%) occurred in two weeks,the 8 cases(20%) after two weeks.In terms of prognosis,12 cases(30%) were basically cured,the condition has improved in 18 cases(45%),improvement in 9 cases(22.5%),1 case(2.5%) of death,the total efficiency of 97.5%(39/40).Conclusion:1.Hypertension,atrial fibrillation,large area cerebral infarction was an important risk factor of hemorrhagic cerebral infarction, Cerebral embolism patients’ incidence of hemorrhagic cerebral infarction was significantly higher than that patients of cerebral thrombosis,large area cerebral infarction patients occurred hemorrhage transformation probability was significantly higher than non patients with large area cerebral infarction.2.Clinical and image manifestations of hemorrhagic cerebral infarction varied from patient to patient,but lack of specificity,lesions located in the cortex.3.The diagnosis of HI requires the clinical features and imaging examination, brain CT and MRI examination is the commonly used method of acute head,CT canprovide rapid diagnosis,the sub acute stage of HI,cranial MRI wsa more accurate.4.After cerebral infarction was hemorrhagic cerebral infarction peak within two weeks.5.The therapy of HI should be individualized,according to the different types of HI and the clinical symptom,imaging findings,Select different treatment options.
