Pathogenesis of normal-pressure glaucoma patients remains unclear since pathogenic condition develops slowly, early symptoms is not obvious patient’s intraocular pressure is maintained within the normal range. Primary open-angle glaucoma contains normal-tension glaucoma, and primary open-angle glaucoma disease process is not clear. Thus it is difficult to diagnose and treat normal-pressure glaucoma and open-angle glaucoma. In this paper, we aim is to explore the differences between primary open-angle glaucoma and normal-pressure glaucoma in optic nerve damage.Firstly, a meta-analysis was utilized to investigate the possibility that diffusion Tensor Imaging(DTI) can detect white matter damage in patients with glaucoma. We search 116 studies. And eleven DTI studies were identified through a comprehensive literature search, and 10 independent DTI studies of glaucoma patients were eligible for the meta-analysis. Our findings revealed that the optic nerve and optic radiation were vulnerable regions in patients with glaucoma and that FA may be correlated with glaucoma severity and age. Furthermore, this study suggests that magnetic resonance imaging in patients with glaucoma may help to provide objective evidence to aid in the diagnosis and management of glaucoma.Secondly, we suppose there is difference between primary open-angle glaucoma and normal-pressure glaucoma in gray matter and white matter change. Thus, we process T1 data to detect the difference by using Voxel-Based Morphometry(VBM), including healthy control group of 20 person(10 males, 10 females), normal-pressure glaucoma group of 15 patients(9 males, 6 females), primary open-angle glaucoma group of 16 patients(10 males, 6 females), the age range from 23 to 75 years old. Analysis results showed that the gray matters and white matters are damage in normal-pressure glaucoma and primary open-angle glaucoma structure on visual pathways. And primary open-angle glaucoma affects the senior visual cortex(BA19 area), normal-pressure glaucoma damages visual pathway of terminal(inferior parietallobule). Thus, we can guess primary open-angle glaucoma damages visual pathways, and then affects BA19 area, and normal-pressure glaucoma damages the inferiorparietallobule, then affects the optic nerve; and the normal-pressure glaucoma and primary open-angle glaucoma is difference in process of morbidity.Finally, DTI data of two types of glaucoma were analyzed statistically to detect FA change by using two sample test methods. Results were concluded that primary open-angle glaucoma and normal-tension glaucoma there were no significant difference in the region of the FA to reduce. And reduce area mainly concentrated in the temporal gyrus, temporal lobe, occipital lobe, occipital lobe gyrus, frontal lobe, primary visual cortex and advanced visual cortex. It can be seen that FA change area and FA reduce have different significantly between two types of glaucoma.Above all, change of primary open-angle glaucoma and normal-pressure glaucoma in optic nerve pathways and visual cortex have significant difference as well as the process of morbidity.
