Objectives The research used CGA(Comprehensive Geriatric Assessment, CGA) in the process of improving the nutrition status of elderly hospitalized patients and explored the influence of nutrition index and the quality of survival of elderly hospitalized patients on the basic clinical diagnosis and treatment. The propose is providing clinical and practical basis to better improving nutritional status, disease prognosis and quality of life in aged patients.Methods Selected 768 hospitalized elderly patients in Affiliated Hospital of North China University of Science and Technology form May 2013 to May 2014 in geriatrics department in the hospital treatment.There are 141 cases which conforms to the inclusion criteria, including 70 male and 71 female. Inclusion criteria:(1) age between 60 ~ 90 years old;(2) miniature nutritional rating scale score less than 23.5;(3) conscious;(4) in the hospital more than 48h;(5)A variety of chronic diseases coexist;(6)people willing to participate in the clinical research.The selected patients were randomly divided into intervention group of 71 cases and a control group of 70 cases.The details of basic information, disease history, family history, personal history, more than 8 hours on an empty stomach before extracting peripheral venous blood checking albumin, albumin,hemoglobin, and lymphocytes coefficient assay index, measure the height, weight, upper arm muscle circumference,diameter of leg,and calculate the BMI(Body Mass Index,BMI)were collected at the beginning of research. All selected patients admitted to hospital within 3 days to complete nutrition risk assessment, comprehensive geriatric assessment,and the evaluation of the quality of life, the control group given conventional treatment,nutritional guidance, nutrition education and nutritional support, intervention group given CGA at the same time, and given scientific intervention for corresponding problems. All the selected patients give four nutrition risk assessment, after 1 month review blood nutrition indicators and review assessment of quality of life after 3 months. A database of all the research data using Excel 2003, application SPSS17.0 statistical software for statistical analysis.Count data expressed as a percentage, between the two groups by chisquare test;Measurement data with mean±SD, itself is for paired samples t-test,comparison between the two groups using rank and inspection, P<0.05 for the differencewas statistically significant, P<0.01 for the difference is significant statistical significance.Results 1 There was no significant difference between the two groups in basic information(gender, marriage, culture degree, occupation, age), anthropometric data(body weight, body mass index, upper arm muscle circumference, diameter of leg),the data of GCA(P>0.05).2 Both of groups in the former albumin, albumin, hemoglobin,and lymphocyte count blood nutrition indicators were no obvious difference(P>0.05),and after the research, both groups on the blood nutrition index also increased to different extent, the intervention group of blood indexes were better than control group(P<0.05).3There was no obvious differences in micro nutrient score(P>0.05), both groups mini nutritional assessment score in 7 days, 14 days and 3 months after all different levels rise,After 3 months of intervention group were higher than that of control group(P<0.05).4Both groups have no obvious differences in survival quality assessment(P>0.05), after research, the quality of life scores of the two groups are also change to increase(P<0.05),and the intervention group in the body pain, social function, energy, and physiological functions of four aspects to improve more obvious(P<0.01).Conclusions 1 In the process of introducing CGA can find potential and closely related with elderly malnutrition problems in clinical treatment of elderly malnutrition.2 on the basis of the traditional clinical application, CGA and intervention can further improve the blood nutrition indicators of elderly patients and improve nutrition risk assessment score,reduce the occurrence of senile inpatients nutritional risk.3 CGA and intervention can improve their quality of life while improving nutritional index in hospitalized elderly patients,especially in the body pain, social function, energy, and physiological functions of four aspects to improve more obvious.
