Objective:The comparative study collected intraoperative and postoperative indicators of primary hepatic carcinoma cases treated in the Affiliated Hos-pital of Qinghai University who were applied with pringle maneuver or hemi-hepatic vascular occlusion in recent years and conducted retrospective analysis.By comparing the effects of two applications of vasc ular occlusion mentioned above,we evaluated and predicted their value in the surgical treatment of primary hepatic carcinoma in Qinghai plateau reg ion.Methods:(1)The research came from the Nationwide multicenter collaborative projects hosted by Wuhan Tongji Hospital Prof. Xiaoping Chen and they provided the patients information collection form.We collected medical records of patients suffered with primary hepatic carcinoma who accepted heaptectomy and applied with pringle maneuver or hemihepatic vascular occlusion during the operation for nearly five years by using the Affiliated Hospital of Qinghai University HIS system and filled form mentioned ab ove.(2) A total of 69 cases of primary hepatic carcinoma patients underwent heaptectomy and applied with pringle maneuver or hemihepatic vascular occlusion during the operation in the Affiliated Hospital of Qinghai University from 2009 to 2013 according to the predetermined selection criteria and 39 patients were applied with intraoperative pringle maneuver(group A),while 30 patients with hemihepatic vascular occlusion(group B). We had a detailed record of the two groups of patients with postoperativ e liver function,postoperative complications,etc.and evaluated the impacts on indicators of two different vascular occlusion methods mentioned above, while performing statistical analysis.Results:We find that the recovery of liver function of group B(eg: AST, ALT,TBIL,DBIL,ALB,etc.) is better than group A(P<0.05); the difference inthe incidence of postoperative complications between group A and B is not statistical significant(P> 0.05).Conclusion:Hemihepatic vascular occlusion applied in hepatectomy c an effectively reduce the damage to the liver caused by hepatic ischemia, better improve postoperative recovery of liver function and effectively avoid liver dysfunction;we are currently unable to confirm the result that hemihepatic hepatic vascular occlusion method can reduce the incidence of postoperative complications.
