Analysis The Epidemiological Characteristics And Disease Burden Of Hospitalized Children For Fall Injury From 2011 To 2013 In Shanxi

Abstract/Summary:
Objective:Study the epidemiological characteristics of fall injury children who were in hospital and analyzation of disease burden.Methods:The subjects were recruited from four hospitals in Taiyuan, Datong, Yuncheng in Shanxi Province by stratified cluster sampling method. These children were selected for hospitalized fall injury during the year of 2011~2013. The data were collected based on the Hospital Information System by trained investigators using unified questionnaires through.The information covers general information including gender, age, address, time of occurrence in a day; clinical classfications: site of injury, nature of injury, degree of injury;disease burden indexs: hospitalized duration, outcomes, and hospitalized expenses. The survey will focus on showing a genaral trend, but not for a case analysis. The data were input and analyzed by using Microsoft Excel 2003 and SPSS 17.0.Results:1. The distribution of hospitalized children for fall injury(1) 1275 person-time was collected into this study. Among the subjects, 840 were male and 435 female, and the average gender ratio was 1.9:1. The number of children in each group was as: 184 in 0 year old group, 509 in 1~4 years old group, 357 in 5~9 years old group, 225 in 10~14 years old group, respectively. With increasing age, the number offall injured children in hospital gradually reduced. There was statistically significance between different boys and girls in all age groups(P <0.05), namely the boys demonstrated higher chance to be injured in this calagory than girls. In particular, the ratio of boys verse girls in the 10~14 years age was highest, i. e. 2.5: 1.(2) There were 710 of them living in farm, while 566 in urban. The average area ratio was 0.8:1.(3) The distribution of different year were 383 in 2011, 438 in 2012, and 454 in 2013,respectively. Increased 71(18.5%) during this 3 years. The average speed of the development rate was 108.9% during 2011~2013, the average growth rate was 8.9%.Average month occurrence was 106.3 ± 20.06. The peak period of falling injury was March to October, especially in March, August and October. There was average 127 in the three month, covers 30.0% of the total numbers. The peak period in 2012 and 2013 was March,May, August and October; in contrast, the peak period in 2011 was January and September.2. The clinical characteristics of hospitalized children for fall injury(1) The sites of injury of hospitalized children for fall injury in different genders, ages,regions, were as following: head and neck, upper limbs, lower limbs. By using Fisher’s exact test, there was statistically significance between different sites by ages(P<0.001). Byc2 test, there were statistically significance between diffrernt sites by regions(c2=16.204,P=0.006).(2) The catagory of injury of hospitalized children for fall injury in different genders,ages, regions, the first rank was S06. By Fisher’s exact test, there were statistically significance between different nature by ages(P<0.001). By c2 test, there were statistically significance between different nature by regions(c2=17.498, P=0.008).(3) The degree of injury of hospitalized children for fall injury in different genders,ages, regions, the first rank was severe injury. By c 2 test, there were statistically significance between different degree by ages(c2=188.446, P<0.001).3. The disease burden of hospitalized children for fall injury(1) Among the children in hospital due to fall injury, the cure rate was 64.7%,improvement rate 32.8%, the fatality rate 0.1%. By Fisher’s exact test, different ages, siteof injury, nature of injury, degree of injury in the distribution of hospitalized outcome were statistically significant(P<0.05). By ordinal logistic regression analysis, moderate injury hospitalized outcome was 0.319 times compared to severe injury.(2) The PYLL was 79.63 person-years and YLL was 0.16 person-years.(3) The indirect economic burden was 795.31 Yuan, the average hospitalized days was9.25 d, the minimum was 1d, the maximum was 165 d. By ANOVA, there were differences in hospitalized days dependend on different ages, regions, site of injury, nature of injury,the degree of injury. By multiple regression analysis, the main factors were the degree of injury and ages.(4) The cost was 11.0175 million Yuan, the average cost was 8641.17 yuan. By analysis of variance, different ages, regions, site of injury, nature of injury, the degree of injury, there were differences in hospital cost. By multiple regression analysis, the main factors were the hospitalized days, and degree of injury.(5) The expected average cost in hospitalized children for fall injury is 8787.38 Yuan in 2014.Conclusions:1. Male, 1~4 years old, rural area children are in the highest-risk based on hospitalized population when occurred fall injury, and the rate increasing quickly. They are the key object for prevention of children fall injury.2. The features of hospitalized children for fall injury were head and neck injury(S06),and severe injury.3. The cure rate of hospitalized children for fall injury was pretty low with long the hospitalized staying days and high cost, which brings heavy burden to individuals, families and society.
