Establishment And Evaluation Of Nursing Outcome Evaluation Index System On COPD

Objective:To develop a more suitable scale of Nursing outcome evaluation index system for Chronic obstructive pulmonary disease(COPD) in China according to the theory of Nursing outcomes classification(NOC), which can be used to provide assessment tools for exploring measures to improve the care of patients of COPD. Method:(1).Through literature method,semi-structural Interviews,focus group discussions(FGD), the Conceptual Framework of scale for NOC was constituted, and then the clauses pool was formed.(2)Through expert consultation, the initial adjustment and modifications opinion of the expert on questionnaire structure and language was collected, and built the database. Then the improved Delphi method was applied to select and amend entries, and the authority of coherence, return rates of the consultation and the harmonious coefficients was evaluated. The final scale for COPD was formed.(3) Through on-site questionnaire investigation, the data of COPD was built. The statistical methods were used to evaluate the reliability, validity, discrimination of the scale. Results:The assessment index system based on NOC with COPD suitable for Chinese people was established,which is first grade assessment indicator 6,second assessment indicator 22, third assessment indicator 79 items containing functional health, physical health, psychological health, health knowledge and behavior health, Perceive health and Family health fields. The functional health field included three dimensions of Endurance, Sleep, and Self-care : activities of daily living,which is amount to 14 indicator.the physical health field included four dimensions of Communication skills, Nutritional status, Lung function status: gas exchange, the severity of infection, which is amount to 12 indicator. the psychological health field included three dimensions of Anxiety, Depression and Desire to survive, which is amount to 11 indicator. the health knowledge and behavior fields included five dimensions of Health beliefs, For-health behavior, Knowledge: health promotion, Risk Control: smoking, Knowledge: infection control, which is amount to 16 indicator.the perceive health field included four dimensions of Quality of life, Severity of symptom, Patient satisfaction: psychological care, Patient satisfaction: instruct,which is amount to 13 indicator.the Family health field included three dimensions of Home care, Family support, Family physical environment, which is amount to 13 indicator.Then the reliability, validity, discrimination and feasibility of the scale were evaluated. The reliability: which were verified by Cronbach coefficient with 0.911 of total scale. The validity: The results showed that the outcome of the measurement scale was meet expectations on the idea of a professional. The discrimination: ICC curve distribution indexes ideal indicators contain large amount of information, the error is small. The feasibility: Sample survey was be carried out in patients with COPD, Scale acceptance rate of 96.5%, 95.9% completion rate, and the average time to answer was be controlled within 20 minutes. Conclusions:The results show that the evaluation index system we have developed has good reliability, validity and feasibility. This evaluation index system can be used as a tool of evaluation studies of the COPD based in NOC.
