Objective To evaluate the status of community residents hypertension,and analyze the impact of environmental factors and epigenetic factors in the prevalence of hypertension, and propose countermeasures of hypertension.Methods Community residents were selected from April to September in 2014.Used the method of field epidemiology combined with experimental epidemiology to do field survey and collect saliva,and analyze the relationship between the DNA methylation of AGTR1 gene and hypertension, and comprehensively evaluate the main factors of hypertension.Result(1)8720 community residents were selected and the valid questionnaires were 8371,the effective rate was 96.00%.The average age of all community residents were 40.76±16.92 years old.(2)The total prevalence of hypertension was 20.9%, and the standardized prevalence rate was 22.5%, the difference of prevalence between male and female was statistically significant. The prevalence of hypertension increased gradually with age, the prevalence of 30 to 45 age group increased more significantly larger, the prevalence of ≥60 age group was highest. 666 patients with hypertension were new detected, 1081 hypertensive patients were previous detected with 513 cases normal blood pressure in this investigation, the control rate was 47.5%. These residents had lower knowledge and awareness of their own blood pressure with 61.3 %.There was significantly statistical difference between men and women, women are more concerned about their blood pressure than men, more than a third of hypertensive patients did not know their blood pressure before the survey.(3)Multivariate logistic regression analysis showed that, after the adjustment of sex, age, education level and marital status, the environmental risk factors of community residents suffering from hypertension, were frequently drinking(OR = 1.253,95% CI: 1.125 ~ 1.395), higher BMI(OR = 2.648,95% CI: 2.326 ~ 3.015) and central obesity(OR = 1.319,95% CI: 1.159 ~ 1.502); there was multiplied interaction between overweight and central obesity in the prevalence of hypertension(P <0.05).(4)The average value of TM was 71.784, and standard deviation was 0.210. After controlling of age, height, weight, waist, hip, there was negatively correlated between the Fujian community residents’ TM values and SBP, DBP(P <0.05);There was significantly statistical difference of TM value between hypertensive patients and control groups(P <0.05). TM values of hypertensive patients were lower than the control groups, which suggested that DNA methylation levels of hypertensive patients was lower than the control groups. Multivariate regression analysis showed that, after the adjustment of demographic and environmental factors, high levels of DNA methylation was a protective factor for hypertension, with TM value increased by 0.1 units, the risk of hypertension reducing 0.652 times. Smoking and eating disorders and low levels of methylation were present multiplied interaction in hypertension(P <0.05).Conclusion(1) High blood pressure of residents in Fujian Province was the result of many factors, which the control rate was better, but residents’ knowledge and awareness of their own blood pressure was low, we need to intensify propaganda healthy lifestyle community, and gradually increase the knowledge of hypertension and their blood pressure awareness.(2)DNA methylation are closely related to the hypertension, we can start from epigenetic aspects in the development of hypertension control measures.
