Part Ⅰ Magnetic resonance imaging of prostate: comparison of image quality using torso and HD cardiac phased array coilsObjective To compare the quality of MR imaging for the prostate between 8 channel torso phased array coil and HD heart in order to improve the image quality of prostate MR routine scan.Methods 54 patients with prostate disease were divided into two groups according to body mass index(BMI)(BMI≤25kg/m2)and matched age, and they were selected to be performed prostate MRI scan with the two kinds of coils respectively. The uniformity,signal to noise ratio(SNR)and contrast to noise ratio(CNR) of the transversal T2-Weighted imaging were analyzed by independent samples t test.Result The imaging uniformity in central gland and the peripheral prostate zone of cardiac phased array coil and torso phased array coil were(0.93±0.03),(0.88±0.05),(0.98±0.02),(0.93±0.04).The imaging uniformity between the two kinds of coils were statistically different(t=3.89,5.30, P<0. 05);SNR and CNR in central gland and the peripheral prostate zone of cardiac phased array coil and torso phased array coil were(144.81±58.00),(151.49±70.92),(186.67±81.10),(194.15±89.37);(102.48±39.75),(100.27±50.03),(139.48±65.07),(142.93±71.80),and there was no statistically difference(t=-0.38,-0.32;0.18,-0.19,P>0.05).Conclusion Cardiac phased array coil made better imaging results in the evaluation index of imaging uniformity, but no obvious difference in the imaging SNR, CNR to the torso phased array coil, to a certain extent, cardiac phased array coil can replace the torso phased array coil in prostate thin-slice and high resolution of conventional MR imaging.Part Ⅱ The influence of scanning effect of the injection flow rate of contrast medium on Magnetic resonance image dynamic contrast-enhanced of prostate cancerObjective To analyze effect of perfusion index of the injection flow rate of contrast medium on Magnetic resonance image dynamic contrast-enhanced(DCE-MRI) of different pathological grading of prostate Cancer in order to determine the best injection velocity.Methods 70 patients with pathology proven PCa、Cardiac and renal function is normal and BMI≤25kg/m2,3.0T MR Liver Acquisition with Volume Acceleration was used.The 35 patients with dynamic enhanced injection velocity contrast agent was 2.5ml/s, the other 35 patients was 5ml/s.All of the data was transferred to GE Advanced Workstation 4.3,the indexes of the peripheral prostate cancerous zone and calculated by Functool2 of signal intensity time(SI-T),the time to minimum(Tmax),the whole enhancement degree(SImax%) and the maximum slope(Rmax) were calculated.And analyzing the effect of different injection velocity on the dynamic enhanced perfusion index.Result Two groups of different pathological grading PCa patients with contrast agent injection velocity 2.5ml/s and contrast agent injection velocity 5.0ml/s low risk group(Gleason score 2 to 6)、Intermediate risk group(7 Gleason score) and high risk group(Gleason score 8 to10) average of three groups of Tmax were(19.89±2.76)s、(15.42±1.68)s;(16.91±2.34)s、(12.88±1.73)s;(14.13±1.81)s、(10.2±1.42)s,The Two groups of Tmax were statistically significant difference(t=4.61,3.1,3.25,P<0.01).Two groups of PCa average of SImax% were(1.45±0.17)%、(1.51±0.27)%;(1.62±0.12)%、(1.84±0.18)%;(1.86±0.16)%、(2.11±0.28)%, Two groups of SImax% were statistically significant difference(t =-2.44,-4.55,-5.16,P<0.05).Two groups of PCa average of Rmax were(6.29±2.62)%、(7.64±4.09)%,(8.92±4.21)%、(10.24±9.09)%,(10.85±2.89)%、(12.43±3.51)%,Two groups of Rmax were statistically significant difference(t =-4.07,-3.85,-8.68,P<0.01).Tmax of prostate cancer patients with contrast agent injection velocity 5ml/s was shorter than velocity 2.5ml/s; SImax% and Rmax of the former was higher.Conclusion Prostate cancer patients with contrast agent injection velocity 5ml/s dynamic enhancement perfusion index more sensitive than velocity 2.5ml/s,it can improve the effect of enhancement of prostate cancer, and to improve the diagnosis of prostate cancer.
