The Smoking Situation And The Related Impact Factors Among Migrant Workers In Jiangxi Province

Objective:Field epidemiological research was applied in 5 counties and areas in Jiang Xi province among floating employed population To learn about the smoking behavior and impact factors,and targeted provide reference for tobacco prevention and control measures.Methods:(1) Sampling method:Six sectors:in five counties(districts) of Jiangxi province were selected using stratified multi-stage cluster sampling, and 1486 migrant workers aged above 18 years old were surveyed.First,5 counties(districts) of manufacturing, wholesale and retail trade, accommodation, catering, social services,construction and other(including farming, forestry, animal husbandry, fisheries, and electric power, heat, gas and water production and supply industry) six large sectors of the migrant workers were extracted by stratified sampling;Second, implemented an equivalent sampling of the industry, namely each site each type of industry surveys of not less than 50 people.(2)Field investigation method: after concentration, respondent were surveyed by face to face interview,according to the survey,the.investigators to fill out the questionnaire(3)Epi Data3.1 software was adopted to establish the database and double entry by double; SPSS16.0 software for data analysis, the mean, rate, ratio of index data for statistical description, the chi-square test, nonparametric test, rank correlation of data into the comparative analysis,Logistic regression to analysis of influencing factors of smoking tobacco, smoking behavior changed by tobacco prices rosing through the analysis of the curves, P values were bilateral inspection results, α= 0.05.Results:(1) The research was carried out among the floating population in Donghu area(Nanchang City), Wuning county(Jiujiang City), Zhanggong area(Ganzhou City),Longnan county(Ganzhou City) and Shanggao county(Yichun City), issued 1500 questionnaires in total, from which 1486 effective questionnaires withd-rawed, the effective rate was 99.1%;According to the investigate information, male female ratiowas 1.29:1,836 was male, 650 was female.(2) Among 1486 respondents, 487(32.8%) current smokers, thereinto, male smokers occupied 56.3%, and female smokers occupied 2.5%. Among the male smokers, smoking ratio of 35-44 years old(60.6%), and primary school or below(67.2%), monthly income below 1000yuan(75.0%), Commercial and service industry workers(31.2%), meanwhile, the smoking ratio of those living in dormitory(56.6%)and married(51.8%) were higher than others.(3) Daily smoking initiation over 20 cigarettes rate was the highest in men(51.6%), while the lowest below 10 cigarettes in women(56.3%). The difference of age, educated levels, marital status and professional between light, moderate and severe men smokers had statically significance.(4) 526 out of 999 non-smokers were suffering from second-hand exposure,the exposure rate was 52.6%, 51.0%, 53.5% of male and female. Difference of second-hand exposure between male and female among age group, housing status and monthly income have statically significance. 72.3% of the population was harmed by tobacco directly or indirectly.(5) 30.2% of those 487 smokers were trying to give up smoking. The quit smoking rate of light and moderate smokers reached to 32.0%, while the ratio of serve smokers was 28.5%;36.3% smokers would giving up smoking after catching the view of health warnings on the cigarettes pack, 45.5% of smokers who see the doctor had got smoking cessation advice from a doctor or health care worker.(6) Single-factor analysis indicated that gender, education levels, housing status,monthly income, professional, drinking conditions, life stress, life satisfaction, ill situation had impact on smoking, After multiple factor analysis,all the variables had significant effect except monthly income, professional and drinking conditions.(7) 43.9% purchased cigarettes with the price per pack ranged from 10 to 20 RMB and maximum of fees on cigarette consumption would reached 1800 Yuan per month,Median cigarettes cost was 200 Yuan per month. Light, moderate cigarette smoking monthly fees were lower than the heavy smokers.(8) When cigarette prices reaching doubled,70% of the smokers would reduce smoking,as cigarette prices rising four times,70% of the smokers would give upsmoking.Conclusion:(1) Men of floating population have widespread smoking behavior, 56.3% were smoking current, and serve smoking population was over 50%, significant higher than the general population in China, but still lower than some developed city communities. Second-hand exposure was the same serious among men and women;more than half were exposed to the damage, hints controled tobacco hardship.(2) High degree of culture, life satisfaction is high, the female is inhibitory factors for smoking, life pressure, working pressure, live in dormitories, in good health10 variants were found to be meaningful in single-factor analysis of smoking.7factors were taking into the multiple-factors analysis,(3) The investigation showed the ratio of smokers, trying to give up in floating population Jiangxi province, was low, moreover, only less than 50% received smoking cessation advices from medical workers. Although most of them have noticed the health warnings on the package, only less than half would consider quitting smoking.(4) Tobacco consumption have aggrandized economic burdens to floating population, and monthly consumption on cigarettes have climbed up to 1/6 of monthly income. Hence, changing smoking behavior of smokers among floating population have a large effect in real disposable per capita income(DPI) and improvement of family life quality.(5) Plot of cigarette prices influence smoking behavior found that cigarette price increase lower multipleswould prompt most smokers to reduce smoking or replace cheap cigarette brand, reasonable raise tobacco sales prices, would change the smokers smoking behaviour.
