Objective To evaluate the clinical characteristics and prognosis of severe encephalitis with respiratory failure, and to explore the related factors for respiratory failure.Methods A retrospective analysis was conducted on 66 severe encephalitis inpatient clinical data from pediatric wards of Affiliated Hospital to Qingdao University, covering from Jan.2009 to Dec.2010.These children were divided into 2 groups(respiratory failure group and non- respiratory failure group). The data were processed by SPSS 13.0 saftware,t test and χ2 test were used to analyze the clinical data,the related factors and prognosis for respiratory failure.Results Forty-six point six seven percent(14/30 cases)of severe encephalitis patients with respiratory failure died or gave up treatment, twenty percent(6/30 cases) were cured, and others had varying degrees of sequelae like epilepsy, insane, dyskinesia,act. The statistically significant factors for severe encephalitis children with respiratory failure were frequent convulsions, coma, hyponatremia, hypocalcemia, hyperglycemia and increased intracranial pressure(P<0.05), while age, sex, mode of onset, fever, headache, pathological syndrome, serum potassium and cerebrospinal fluid biochemistry had nothing to do with respiratory failure in children with sere encephalitis.Conclusions Severe encephalitis with respiratory failure is a serious threaten to children’s life and quality. Frequent convulsions, coma, increased intracranial pressure, hyponatremia, hypocalcemia and stress hyperglycemia may be risk factors for respiratory failure in children with sere encephalitis.
