Objective:To compare the corneal refractive condition and operation effect within 3 months of modified 20-gauge vitrectomy and 23-gauge vitrectomy for various vitreoretinopathy.Methods:A prospective and random case series study.This 42 cases who need surgical treatment divided into the conventional 20-gauge pars plana vitrectomy group(20 eyes,20 G group) and the 23-gauge transconjunctival sutureless vitrectomy group(22 eyes,23 G group). BCVA,NCT, were recorded preoperatively and postoperatively at 1 day,1 week,2 week,1 month and 3 month.Ave K,Cyl,SRI,SAI of paients were recorded preoperatively and postoperatively at 1 week,2 week,1 month and 3 month..Operation time,age,gender,eye were recorded too.Using t test,chisquare test and analysis of variance of repeated measurement data to statistical analysis.Result:In this study,the modified 20 G group average operation time was(71.20 + 17.20) min and the 23 G group average operation time was(58.14±10.78) min.The operation time in two groups is statistically significant.At last follow-up,there was 21 eyes(95.5%) and 17 eyes(85.0%) with visual acuity ≥0.05 in 23 G and modified 20 G groups.Compared with preoperatively,both groups is statistically significant.But there was no statistical difference between two groups.The modified 20 G group did not appear low intraocular pressure.The early stage of postoperative,hypotomy was detected in 3(13.6%) in 23 G group,there was no statistical difference between two groups. Postoperatively high intraocular pressures was detected in 9(45%) in modified 20 G group and 8(36.4%) in 23 G group.There was no statistical difference between two groups in postoperative high intraocular pressures(χ2=0.324,P>0.05).Two groups of patients’ intraocular pressure were correlation between different time points(P<0.1); There was correlation between intraocular pressure changes and time(F=7.457,P<0.05). There was no interaction in time and group(F=2.619,P>0.05).The overallmean of intraocular pressure in both groups were equal(F=0.667,P=0.419).Compared with preoperatively,the Ave K at the first week after operation in modified 20 G group was statistically significant(t=-2.053,P<0.05)and the Ave K at the first week after operation in 23 G group was no statistical difference(P>0.05).Compared the Cyl at the second week after operation, there was statistically significant between modified 20 G group and 23 G group( t=3.31, P<0.05). Compared with preoperatively,the Cyl at the first week after operation in 20 G and 23 G group was statistically significant. Compared with preoperatively, the Cyl at the second week after operation in modified 20 G group was statistically significant(t=-4.86,P<0.05)and was no statistical difference in 23 G group(t=-1.91,P>0.05).In two groups,there were statistically significant in SRI compared with those preoperatively at the first and second week after operation(P<0.05).,but there were no statistical difference between modified 20 G group and 23 G group(P>0.05). Compared the SAI at the first week after operation, there was statistically significant between modified 20 G group and 23 G group(t=2.64,P<0.05). Compared with preoperatively, the SAI at the first week after operation in modified 20 G group was statistically significant(t=-3.91,P<0.05)and was no statistical difference in 23 G group(t=-0.16,P>0.05).Conclusion:1.The operation time in 23-gauge micro-incision vitrectomy was effectively shorter than modified 20-gauge vitrectomy.And less effects on ocular surface.2.There are not significant difference in BCVA,NCT,SRI between23-gauge vitrectomy and modified 20-gauge vitrectomy.3.The same effect in minimally invasive could also be obtained by the modified 20-gauge vitrectomy...
