Objective To analyze the implementation and effect of clinical pathway on neonatal respiratory distress syndrome (NRDS).Methods Research object were selected from children hospitalizated with NRDS in Neonatal Center of Children’s Hospital of Chongqing Medical University from 2011 to 2014,Neonatal hospitalizated with NRDS applying pathway management from January 1,2013 to December 30, 2014 were pathway management group, patients hospitalizated with NRDS non-using pathway management from January 1,2011 to December 30, 2012 were non-pathway management group. The expenses of hospitalization,length of hospital stay,respirator using time,incidence of ventilator associated pneumonia(VAP),hospital infection and the satisfaction of children’s parents were evaluated between two groups,and analyzing the variation of clinical pathway.Results There are 1074 eligible cases in total, including 494 cases for clinical pathway management group and 580 cases for non-pathway management group. There were no significant difference in age, gender, gestational age, birth weight, chest radiograph classification between two groups (P> 0.05). Hospital total cost, medicine cost, examination fee, inspection fee, nursing fee, treatment cost decreased and length of hospital stay, respirator using time shortened after pathway management, besides occurrence of ventilator associated pneumonia improved too. But there were no significant difference in hospital infection and the satisfaction of children’s parents (P> 0.05). Among clinical pathway patients,159 patients completed the clinical pathway, variance was found in 315 cases. The main reason for variation were complications of NRDS, and patent ductus arteriosus (PDA) is the most common complications.Conclusion For neonatal with NRDS, clinical pathway management has an effect on cutting down length of hospital and expenses of hospitalization, besides ensuring efficacy and satisfaction of children’s parents. Because of complications of NRDS, the mutation rate is high. It still need improving and then applie to clinical work better.
