The Treatment Of Femoral Bone Nonunion By Single Arm External Fixator Conbined With Autologous Bone Transplantion In Children

Objective:To investigate the curative effect for patients of femoral bone nonunion by single arm external fixator combined with autologous bone transplantation in children.Methods:There are 36 patients who with femoral bone nonunion of our hospital during the period 2006.4-2013.12,which treatement with residual epiphyseal lesion clerance,recanalization of the medullary cacity,autologous bone grafts,and fixation with single arm external fixator were retrospectively analyzed.Results:All cases of 36 patients were followed up for 1 year to 7 years. The sample included 20 males,16females, age ranged from 4 years old to 12 years old.. According to improved AS AMI score system,21 patients were evaluated as excellent,9 patients as good,4 patients as fair, and 2 patients as poor. All cases received synostosis. For one case, osteotylus was not approving 6 months after operation; by 3 times of autologous bone marrow transplantation, and adjustment of external fixator, synostosis was received after 30 months. For another case, refracture occurred after demolition of external fixator and beginning walking; external fixator combined with autologous bone grafts were applied, and synostosis was received after 36 months, with good function of knee joint and hip joint.For 1 case during the follow-up has not yet healed.Conclusions:Single arm external fixator wear lightweight, easy to care, the operation is relatively simple surgical operation time is short, simply remove the patient tolerated,which promoted the repairing of focus of infection and the toleration of patients.Autologous bone has reproducibility,conductibility, inducibility,and no immunogenicity.Single arm external fixator combined with autologous bone grafts for the treatment of femoral bone nonunion imoroved cure rate. It is an ideal choice for the treatment of femoral bone nonunion in children.
