Background:Wet age-related macular degeneration is the main reason for over 55 years of severe vision loss in patients in the current. Which is characterized by subretinal retinal pigment epithelium layer active choroidal neovascularization formation.Research shows that in exudative age-related macular degeneration, the inflammatory lesions and growth factor expression and synthesis.The vascular endothelial growth factor plays a vital role in the formation of choroidal neovascularization.More and more attention is focused on the anti-VEGF treatment.Intravitreal injection of anti-VEGF drug ranibizumab can bind VEGF-A, inhibit the formation of choroidal neovascularization, reducing macular edema, become the most safe and effective method.Purpose:Analysis of changes in exudative AMD patients VEGF concentration in aqueous humor before and after ranibizumab treatment.Observe before injection and month after injection the changes in retinal morphology and measuring subfoveal macular retinal thickness changes by OCT.This describes VEGF involved in the formation and development of CNV.Method:We collected eleven wet AMD patients and sixteen patients with cataract surgery.Eleven patients received intravitreal injection of ranibizumab therapy.Aqueous humor samples collected on the day of injection and one month after injection, using the ELISA test the concentration of VEGF in the aqueous humor.Results:Ranibizumab in all patients prior to injection of aqueous humor concentration of VEGF were significantly higher than the control group (P <0.001).After intravitreal injection of 4 weeks, VEGF concentration in the aqueous humor of patients decreased significantly (P<0.001). Subfoveal retinal macular thickness decreased significantly (P<0.001).Conclusion:Vascular endothelial growth factor is releted to the pathogenesis of neovascular AMD.Intravitreal injection of ranibizumab inhibit angiogenesis, resulting in significant reduction in intraocular VEGF concentration, but not completely neutralize VEGF.After injection OCT showed macular edema and RPE highly reflective improved significantly.The average thickness of the macula subfoveal retinal thickness decreased significantly.These are associated with decreased VEGF concentration in the aqueous humor was positively correlated.This suggests that the level of VEGF is an important prognostic factor.Not completely inhibit VEGF may cause CNV recurrence.Can VEGF inhibition to a lower level is a key factor in successful treatment.VEGF levels were monitored after the injection, the patient’s medication and dose interval may be adjusted in one direction ranibizumab individualized treatment of AMD.
