| Objective:To analyze the relationship between the miR-210 expression and clinicopathological characteristic and explore its clinical significance to the early diagnosis of CRC through testing the miR-210 expression in the serum of Colorectal cancer patients.Method:1. To tested the miR-210 expression of 60 CRC patients (experimental group) and 60 healthy subjects(control group) through the reverse transcription quantitative real-time PCR, and analyze the relationship between patient’s miR-210 expression and their clinicopathological characteristic.2. Through the postoperative follow-up,30 postoperative recurrence patients were chosen from the experimental group. Their serum miR-210 expression was measured one month after the operation and when recurrence. By which, the dynamic change of miR-210 expression was analyzed according to its difference.3. To measure the level of serum CA199 and CEA of the two groups and draw the ROC curves of miR-210, CA199 and CEA by using the automatic electrochemichal immunoassay analyzer, then the area under the curve(AUC) for CRC diagnosis was achieved respectively. According to the Youden index, the diagnosis critical value of miR-210 was achieved by employing the ROC curve of miR-210.4. According to the diagnostic reference value of CAE and CA19-9 kits and the diagnosis critical value of miR-210 obtained by the experiment, the number of true positive and false positive cases in 120 subjects was calculated respectively, namely the value of a, b, c and d. By using formulas:sensitivity=a/a+c, specificity=d/b+d, positive predictive value= a/a+b, negative predictive value= d/c+d, accuracy rate= (a+d)/(a+b+c+d), the sensitivity, specificity, ppv, npv and accuracy rate of CEA, CA19-9 and miR-210 diagnosing CRC were calculated respectively, through which, the diagnostic efficiency of CEA, CA19-9 and miR-210 was compared.Result:1. Through measuring the expression levels of miR-210 in serum of patients group and control group, the result was that the miR-210 expression level of experimental group is observably higher than the healthy controls (P<0.0001). There existed the significant relationship between the expression levels of miR-210 in serum of patients and the tumor size, infiltration degree, lymphatic metastasis and clinical TNM stage. P value was calculated with the statistical significance as 0.037,0.031,0.001 and 0.003 which less than 0.05. There had no relationship between the expression levels of miR-210 in serum of patients and their gender, age, tumor type and differentiation degree, P>0.05.2. The expression levels of miR-210 in serum of 30 postoperative recurrence patients were measured one month later from the laparoscopic assisted radical resection for CRC, which was significantly lower than the preoperative, P<0.001. However, the serum miR-210 expression level measured when local recurrence and distant metastasis in the postoperative follow-up was observably higher than the expression of one month postoperative, P<0.001.3. Through drawing the ROC curves of miR-210, CEA and CA199 the area under the curve (AUC) was achieved respectively as 0.825,0.662 and 0.613. According to the Youden index, the diagnosis critical value of miR-210 was 33.57. Regarding the AUC value, it could be concluded that miR-210 was more accurate than CEA(Z=2.601,P<0.01) and CA19-9(Z=3.253, P<0.01) for diagnosing the CRC.4. The number of true positive and false positive cases in 120 subjects was calculated respectively by using the critical value of diagnosis, namely the value of a, b, c and d. By employing formula, the sensitivity, specificity, ppv, npv and accuracy rate of CEA, CA19-9 and miR-210 diagnosing CRC were calculated respectively. In diagnosing the CRC, the diagnostic specificity, sensitivity, positive predictive value, negative predictive value and accuracy rate of miR-210 were 78.3%,73.3%,75.9%,74.2%and 75%, respectively, which was higher than the CEA, sensitivity 60%, specificity 66.7%? ppv 64.3%, npv 62.5%, accuracy rate 63.3%, and the CA19-9, sensitivity 50%, specificity 65%,ppv 58.8%, npv 56.5%, accuracy rate 57.5%. Therefore it was concluded that the diagnostic efficiency of miR-210 was higher than CEA and CA19-9 in diagnosing the CRC.Conclusion:There is high expression of the circulation miR-210 in CRC patients. And testing the expression level is helpful to improve the diagnostic rate of CRC, which is further expected to be the new noninvasive molecular markers in the diagnosis of CRC. |
