Analysis Of The Value And Safety Of Para-Aortic Lymphadenectomy In The Treatment Of Early Endometrial Cancer

Objective: Systematic pelvic lymph node + abdominal paraaortic lymph node dissection in endometrial carcinoma, especially in the early stage of endometrial carcinoma, still has a great controversy. The current study aims to investigate the value and safety of abdominal paraaortic lymph node dissection in the treatment of early-stage endometrial carcinoma. Methods: The clinical data of 109 patients with early-stage endometrial carcinoma initially treated in our hospital from January, 2010 to December, 2012 were retrospectively analyzed. In addition, all patients were divided into 2 groups according to different ranges of lymph node dissection: group A(only undergoing pelvic lymph node dissection) and group B(undergoing pelvic lymph node + abdominal paraaortic lymph node dissection). Moreover, postoperative complications, recurrence and metastasis of both groups were compared. Results: Among the 109 patients, recurrence and metastasis were found in 12 patients, with 10 in the group A and 2 in the group B; and the recurrence and metastasis rate in the 2 groups was 17.86% and 3.77%, respectively, showing a statistical difference(P = 0.019). The incidence of postoperative complications in the 2 groups was 19.64% and 26.41%, respectively, demonstrating no statistical difference(P = 0.401). Further, there were no statistical differences in the incidence of various complications(lower limb edema, lymphatic cyst, venous thrombosis, low intestinal obstruction, and urinary retention) between the two groups(P > 0.05). Conclusion: Systematic pelvic lymph node + abdominal paraaortic lymph node dissection is able to reduce the recurrence and metastasis rate in patients with early-stage endometrial carcinoma and improve prognosis, and is also proved to be safe and feasible.
