Objective:To explore the risk factors of local relapse and distant metastasis after radical resection of lung adenocarcinoma. Methods:A total of 102 patients with lun ist adenocarcinoma operated in First Affiliated Hospital of Xinjiang Medical Univer sityfrom January 2005 to January 2010 were collected.The correlation between clini copathological characteristics and prognosis was evaluated by single-factor and mult i-factor analyses.The survival curves were plotted using Kaplan-Meier. Statistical dif ferent was tested using Log-rank test. Multi-factor analysis of prognostic factors wa s produced by COX regression proportional hazards model. Results:In the whole gr oup1,2,3 and 5 year Disease-free survival(DFS)were 74.30%,67.30%,54.90% and 44.9% respectively,and the median disease-free survival was 30 months. Single-factor a nalysis showed that tumor size(χ2=9.951,P=0.002), clinical type(central or peripher al)(χ2=8.460,P=0.004), differentiated degree(χ2=4.807,P=0.028), lymph node metast asis(χ2=40.516,P=0.000), pathological stage(χ2=38.769,P=0.000) were prognostic fa ctors for local relapse and distant metastasis in postoperative patients with lung ade nocarcinoma. Muti-factor analysis showed that tumor size(χ2=5.082,P=0.024),differe ntiated degree(χ2=10.467,P=0.001),lymph node metastasis(χ2=4.096,P=0.043) were i ndependent prognostic factors for local relapse and distant metastasis in postoperati ve patients with lung adenocarcinoma.Postoperative adjuvant chemotherapy is the re lated affecting factors for the survival rate of 2 years(χ2=4.488,P=0.034).Conclusion:Tumor size, differentiated degree and lymph node metastasis are independent progn ostic factors for local relapse and distant metastasis in postoperative patients with l ung adenocarcinoma.Postoperative adjuvant chemotherapy is the protective factor for patients with local lymph node metastasis, and in the later pathological stage.
