Efficacy And Adverse Reactions Of Nedaplatin Combined With Paclitaxel Plus Intensity-modulated Radiotherapy On Advanced Cervix Cancer

Objective: To observe the efficacy and toxicity of nedaplatin combined with paclitaxel plus radiotherapy in treatment of advanced cervix cancer.Methods: Choose 60 cases of cervical cancer patients from stage IIB to IVA from Oncology Department of a hospital, from March 2009 to 2011 march. Then, randomly divided into nedaplatin group and cisplatin group, 30 cases in each group. The cervical cancer patients had not received any anticancer therapy before treatment, no chemotherapy contraindications. The expected survival time more than 6 months, and no statistical significance in age, pathological type and FIGO stage clinical data differences. Two groups of patients were treated with intensity-modulated radiotherapy,The external irradiation dose of radiotherapy was2 Gy per time,5d/W, with a total dose of 50 Gy;192 Ir high- dose- rate after loading unit was adopted for branchytherapy one week after external radiotherapy,at a dose of5 Gy per time in A point for 4-5 times. On the first day of radiotherapy chemotherapy and nedaplatin group by paclitaxel 30 mg/m2 of Nedaplatin combined with 20 mg/m2 scheme chemotherapy, cisplatin group take paclitaxel 30 mg/m2 plus cisplatin 20 mg/m2 scheme. Taxol for 12 h and 6 h respectively by oral administration of dexamethasone 20 mg, 30 min before intramuscular injection of isopropyl diazoxide and intravenous injection of cimetidine and chemotherapy plus with antiemetic, protecting stomach and other processing. Two groups were 1 times a week, a total of 4-6 cycle.Results: 1The short term effect In nedaplatin group, the complete remission(CR) in 18 cases, partial remission(PR) in 10 cases, progressive disease(PD) in 2 cases, short-term effective rate of 93.3%.In cisplatin group, the complete remission(CR) 15 cases, partial remission(PR) in 12 cases, 3 cases of progressive disease(PD), the short-term effective rate was 90.0%, there was no significant difference in masses(P =0. 423). 2The long term effect 1, 2, and 3 years survival rate of Nedaplatin group was 90.0%, 72.8% and 56.6%, 1, 2, and 3 years survival rate of cisplatin group was 83.3%, 65.3%, 49.0%, two groups overall survival rate difference was not statistically significant(P = 0.881); median progression free survival time was 28 months in the nedaplatin group, cisplatin group was 22 months. There was no statistically significant difference(P = 0.352). 3The adverse reactionsNedaplatin group occurred leukopenia was 26 cases, cisplatin group was 21 cases, the difference was not statistically significant(P=0.117); nedaplatin group had thrombocytopenia in 23 cases, cisplatin group was 21 cases, the difference was not statistically significant(P=0.559); nedaplatin group had anemia in 21 cases, cis platinum group for the 19 cases, there were no significant difference between them(P=0.584); nedaplatin group had nausea and vomiting in 21 cases, cisplatin group was 28 cases, the difference was statistically significant(P=0.020); nedaplatin group had renal damage in 1 cases, cisplatin group was 1 cases, the difference was not statistically significant(P= 0.754); liver function damage of Nida’s platinum group was 1 cases, cisplatin group was 2 cases, the difference was not statistically significant(P=0.500); nedaplatin group for 15 cases of radiation proctitis, cisplatin group was 14 cases, the difference was not statistically significant(P=0.796); nedaplatin group radioactive bladder Cystitis in 1 cases, cisplatin group was 2 cases, the difference was not statistically significant(P=0.500).Conclusion: Nedaplatin Combined with paclitaxel synchronous transfer of intensity modulated radiation therapy in the treatment of advanced cervical cancer, compared to cisplatin and paclitaxel, no difference in efficacy, but in the gastrointestinal tract reaction have improved significantly, patient compliance is good.
