Objective: 1. To identify whether the uterine contractions, serum OT and PGF2 a level in patients with endometriosis was higher. 2. To examine the effects of atosiban, given before transfer of frozen-thawed embryo to women with endometriosis.Methods: It is a prospective, randomized clinical control trial. 120 women with endometriosis undergoing frozen-thawed embryo transfer(FET) were randomly allocated into the atosiban treatment and the control groups. Another 120 women had infertility due to tubal factor were enrolled into tubal factor group to compare for serum OT, PGF2α levels and the uterine contractions with endometriosis group. In endometriosis treatment group, a single bolus(6.75 mg, 0.9 ml/vial) of atosiban was administrated before embryo transfer(ET). Written informed consent was obtained from each patient before inclusion. The study was approved by the institutional review board.Results: Serum oxytocin(1.89±0.33 versus 1.66±0.32, P<0.01), prostaglandin(PG)F2α(2.83±0.34 versus 2.36±0.35, P<0.01) level and uterine contractions(2.5±1.2 versus 1.8±1.0 wave/min, P<0.01) in the endometriosis group were all significantly higher than the tubal factor group. The clinical pregnancy rate(PR) per cycle and implantation rate(IR) per transfer were 58.3% and 41.0% in the atosiban treatment group, which were significantly higher than the control group(38.3% and 23.4%, P=0.044 and P=0.004 respectively).Conclusions:Women with endometriosis showed higher serum OT, PGF2α level and uterine contractions. The atosiban treatment before embryo transfer in endometriosis is effective in the priming of the uterus suitable for embryo implantation. This is the first study to evaluate atosiban treatment effect in patients with endometriosis.
