Objective The aim of this study was to explore the possible pathogenesis of preecalmpsia with fetal growth restriction,through detect the differences of free fatty acid(FFA) in maternal serum and the different expression of chop, caspase-12 in their placenta.Methods All the 80 patients were admitted to the third affiliated hospital of Zhengzhou University for delivery from September 2014 to May 2015. Inchuded 50 women with preeclampsia,according to wether the neonatal birth weight is normal, divided into preeclampsia(PE group), preeclampsia with fetal growth restriction group(PE + FGR group). The other 30 cases of normal pregnant women as control group. The levels of FFA in maternal serum were measured by the modified copper reagent colorimetric method.Immunohistochemical method was used to explore differences for endoplasmic reticulum characteristic apoptosis factor chop, caspase- 12 in the placenta tissue. RT-PCR assays were performed to determine the transcription levels of chop、caspase-12 m RNA in placenta.Results(1) The average level of maternal serum FFA was(569.45±24.03)mmol/L in PE group and(696.77±71.73) mmol/L in PE+FGR group,were significantly elevated as compared to(452.84±63.87) mmol/L in control group, among the three groups in pairwise comparisons differences were statistically significant(P<0.05).(2) The AOD of chop in placental tissue was 0.178±0.005 in PE group and 0.357±0.019 in PE + FGR group, were higherthan those of the control group 0.041±0.005,(P< 0.05), The AOD of caspase-12 in placental tissue was(0.182±0.010) in PE group and 0.357±0.019 in PE + FGR group, were higher than those of control group 0.073±0.007, were statistically significant among the three groups in pairwise comparisons differences(P< 0.05),(3) The transcription level of chop m RNA in the placenta tissue was 2.022± 0.672 in PE group and 4.137±1.898 in PE + FGR group; respectively higherthan those of the control group 0.998±0.669, among the three groups in pairwise comparisons differences were statistically significant(P< 0.05); The m RNA transcription level of caspase-12 in the placenta tissue was 1.615±0.777 in PE group and 4.177±1.834 in PE + FGR group; respectively higher than those of the control group 1.000±0.778, among the three groups in pairwise comparisons(P< 0.05);(4) The transcription levels of m RNA and protein expression of chop and caspase-12 in placenta of PE+FGR group, were higher than those in PE group, the difference was statistically significant(P< 0.05);(5) In PE and PE+FGR group,the levels of chop and caspase-12 m RNA in placenta was a postive correlation with the levels of serum FFA, were statistically significant(P< 0.05).Conclusions(1) The serum FFA levels in PE and PE+FGR group of pregnant women were significantly higher than the control group, and the serum FFA levels was higher than that of PE group.Elevated FFA levels in maternal serum may be one of the most important facorts of preeclampsia with fetal growth restriction;(2) The transcription levels of chop,capase-12 m RNA and protein in the placenta tissue of PE group and PE+FGR were higher than in the control group significantly, hints of endoplasmic reticulum stress mediated sertoli cell apoptosis may be another important mechanism of preeclampsia with fetal growth restriction;(3) The transcription levels of chop and capase-12 m RNA in the placenta tissue of PE group and PE+FGR were postive correlated with the level of serum FFA,obviously increased the serum FFA levels,finally lead to the increase of endoplasmic reticulum stress, cause apoptosis,cause preeclampsia and fetal growth restriction.
