Objective To compare the positioning type acetabulum file with the normal acetabulum file on the clinical curative effect of acetabulum reconstruction surgery of deformation acetabulum.It can provide theoretical basis for reliable appliance on late acetabulum reconstruction surgery of deformation acetabulum in THA,and spread in clinical.Methods By using the retrospective analysis method.52 patients(63 hips) with deformation acetabulum which collected in the clinical department from January 2009 to June 2014 were divided into the positioning type acetabulum file and the normal type acetabulum file to complete Total Hip Arthroplasty(THA).Divided into two groups, group A(After January 2011) 27 patients(33 hips) and group B(Before January 2011) 25 patients(30 hips);Compared with amount of intraoperative blood loss, time of filing a acetabulum, length of incision; result of postoperative follow-up of 1 year;Follow-up included: chief complaint, physical examination, X-ray radiography( prosthesis position), hip joint function score and guidance joint function exercise etc.All statistical measurements, the mean ±standard deviation( ±S) expressed the quantitative data.The quantitative data used t-test(two independent sample t-test between groups, group repeated measures design analysis of variance), the qualitative data used X2-test(paired design 4 X2-test table data, group design line multiplication list X2- test data);The two sets of data measured value were analyzed. Statistical analysis(P < 0.05), the difference was statistically significant.Results Analy statistical data, and assess results.In the positioning type acetabulum file group,intraoperative blood loss(235.1 ±115.32) ml, file a acetabulum time(8.5 ± 3.44) min, incision length(11.3 ±1.31) cm,while in the the normal type acetabulum file group, intraoperative blood loss(352.6 ±153)ml, file a mortar time(20.0 ±3.61) min, incision length(13.6 ± 2.18) cm.The two different type acetabulum file was statistically significant(P < 0.05) in the intraoperative blood loss, file a acetabulum time, incision length difference. 52 cases acquired follow-up, followed up for 1 year, we completed function Harris hip score, evaluated the surgical effect and compared with them in each time node. Two groups of patients by grading preoperative Harris score, the positioning acetabulum file group was(42.19±5.713) points, and the normal acetabulum file group was(40.78±5.673) points.There was no statistical difference(P > 0.05) in the two groups after analysing statistical data,and had comparable. After the two different surgical instrument operation, we completed function Harris hip score in postoperative follow-up of 3 months, 6 months, 1 year.In the positioning type acetabulum file group, postoperative 3 months(73.2±4.21), postoperative 6 months(83.5±3.21), postoperative 1 year(96.3±4.12),while in the normal type acetabulum file group, postoperative 3 months(62.3±2.13), postoperative 6 months(80.2±3.14), postoperative 1 year(89.8 ± 1.87).The two different type acetabulum files were statistically significant(P < 0.05) after not only grading postoperative hip joint function score but laso preoperative and postoperative hip joint function score.Conclusions The positioning type acetabulum file in THA of the deformation acetabulum can reduce in amount of intraoperative blood loss, time of filing a mortar, length of incision.Meanwhile.easy to use; accurate to position center of rotation;fast to file a mortar and better to improve Joint function. It is the reliable appliance on late acetabulum reconstruction surgery of deformation acetabulum in THA,and it is worth to spread in clinical.
