Background: China is second largest high tuberculosis(TB) burden countries, India being the largest. The number of TB has been ranked among the top Class-A and Class-B infectious diseases and death number of TB patients up to 130000 every year.The aging of population in China become more and more serious with the development of social economy and health level.The elderly over the age of 60 in our country which constituting 14.9% of the total population is as high as 212 million in 2014.The prevalence of active pulmonary tuberculosis was gradually increase by age,peaked in 75 ~ 79 years old, as of 1541/100000.According to the 5th national tuberculosis epidemiological survey in 2010,in patients with active pulmonary tuberculosis, the proportion of over 60 elderly patients is as high as 48.8%, and smear-positive pulmonary tuberculosis patients over 60 with any resistant rate reached 39.7%. Obviously,the old patients with tuberculosis should not be ignored in the link of China’s TB control measures. Some researches in recent years has shown that people often have chronic malnutrition which is related to the development and prognosis of chronic obstructive pulmonary disease, heart failure and some other age-related diseases.Tuberculosis is a chronic wasting disease and tuberculosis patients often have low protein malnutrition which has a bad influence on prognosis.At present, there are a lot of researches about elderly pulmonary tuberculosis disease both at home and abroad,but there is serious shortage of attention to nutritional problems about elderly patients with tuberculosis.There is no systematic research, rarely make the news related clinical data,and there is no corresponding judgment standard and the support scheme applied in clinical nutrition.The purpose of this study is to improve the understanding of elderly tuberculosis patients nutrition status and guild the clinical treatment through the analysis of the relevant nutrition indicators of elderly tuberculosis patients.Objective: To investigate the relationship between malnutrition and the disease and find out the reasonable nutrition evaluation index through the analysis of elderly tuberculosis patients nutrition indicators such as serum albumin, hemoglobin and so on.Methods:The nutritional status of 536 cases of elderly pulmonary tuberculosis patients treated with newly diagnosed which include the serum total protein(STP),albumin(ALB), globulin(GLB), A / G ratio, total number of red blood cells(RBC), hemoglobin(HGB) and lymphocyte(L) was observed in Anhui Thorax Hospital during from January 2014 to March 2015. The reduced profile of each nutrition indicator in patients were statistical analyzed firstly and the relationship between lymphocyte and serum albumin was analyzed in the meanwhile. All the patients were divided into first-treat group and re-treat group, then the patients in each subgroup were grouped again according to the situation of sputum bacterium and gender.At last the comparison and statistical analysis of related indicators were done according to the above group.Results:Albumin in 230 cases(42.91%) was reduced among 536 cases of elderly pulmonary tuberculosis patients, which is the highest reduced indicator of all.Lower hemoglobin in 221 cases(41.23%),A/G ratio was reduced in 148 cases(27.61%),Lymphocytes was reduced 134cases(25%).The level of lymphocytes and serum albumin were positively correlated in all patients.The reduced level of albumin,A/G,HGB and lymphocyte in patients with positive sputum bacteria was more obvious with statistically significant(P<0.05) both in first-treat group and re-treat group. The reduced level of albumin and HGB in patients with positive sputum bacteria was also more obvious with statistically significant(P<0.05) in all male patients and female re-treat patients.Conclusion:Widespread malnutrition exists in elderly patients with tuberculosis, especially in the patients with positive sputum bacteria.Albumin, hemoglobin, A/G ratio and lymphocytes can be used as evaluation indexes to apply in clinical nutrition and guide the treatment.
