Objectives: In order to solve the problem of nursing staff that difficult to quickly and accurately establish intravenous access for the patients in the field without light illumination environment or with poor vascular conditions, developed a small volume,light weight, clear imaging and no need power of venipuncture auxiliary device, the device can clearly show the direction of vein, play an important role in military and civilian fields. Under the premise of using the self-developed apparatus are used in more than a live fire exer cises forces, Air Force General Hospital, Navy Force General Hospital and Beijing Shijingshan District community health service stations, to carry out simulation study on clinical application of dark environment and shock patients.Statistical and comparison of the device in the clinical application effect and to improve the success rate of venipuncture of the nursing staff to provide a new way.Methods: By referring to domestic and foreign literature review and in-depth understanding of medical engineering related theoretical knowledge, used the principle of absorption peak near infrared ray high specificity using deoxygenated hemoglobin in human blood, through the photoelectric conversion and image processing, developed the venipuncture auxiliary devices under field operations conditions. The device is composed of infrared emitter, infrared receiver and liquid crystal display which can assist nurses successfully establishing venous access for patients in any environment,and commissioned by the people’s China device of the PLA General Logistics Department of Health Institute for drug and instrument control of technical index test and test items including appearance, basic performance, high(low) temperature test,reliability and safety testing, vibration testing and deployment withdraw time, detection parameters meet the requirement. After the completion of the technical index test, the device of two parts with clinical application research of representative, one is in dark environment vein puncture auxiliary device of clinical application of part of the simulation, statistical infusion in patients with a success rate of puncture; the second is in shock patients with venous puncture auxiliary for the part of clinical research, shock patients were divided into test group and control group, compared two groups of patients with a puncture success rate and establishing a single venous access time,obtained the results of clinical trials.Results: Developed venipuncture auxiliary device under field conditions, the device has the advantages of small size, light weight, no need of external power supply support,easy to carry, through performance testing, in full compliance with the design and operational requirements, could assist nursing staff successfully venipuncture under the dark environment or patients with poor vascular conditions. Clinical trials of venipuncture auxiliary device carried out in the dark environment simulation device after the successful development of the 30 cases of transfusion patients in the simulation,the dark environment, the nurse with the naked eye cannot see the patient and vein,failed to complete the intravenous transfusion; application of auxiliary device for vein puncture under field conditions, 30 cases of infusion in patients with successful 29, 1cases were not successful, the success rate of puncture was 96.67%; clinical trials of venipuncture auxiliary device for shock patients, 79 cases of shock were randomly divided into control group and experimental group, 42 patients in the control group with33 cases of a puncture, one-time puncture success rate was 78.57%; puncture failed in 9cases, one-time failure rate was 21.43%. In the experimental group, 35 cases of 37 cases were successful, one-time puncture success rate was 94.59%, puncture failed in 2 cases,one-time failure rate was 5.41%. The c2 test, test group disposable puncture success rate was significantly higher than that of the control group, the difference is statistically significant(P < 0.05); control group a puncture success of 33 cases of patients with with established for average operating time(107.61±45.94)s, the test group puncture success in 35 patients with established for average operating time(73.63±29.35)s, by t test, the experimental group established average operating time lower than that of the control group, the difference is statistically significant(P < 0.01).Conclusion: The venipuncture auxiliary devices used infrared imaging technology,through accurate signal conversion system, enable the hand vein presented on the display in real time dynamically. This device is small size, easy to carry and suitable for any environment. After clinical application show that the device performance is stable,the effect of imaging clear. The one-time puncture success rate and the establishment of average operating time were greatly improved by using this device, the device is worthy of further popularization and use, and then has broad prospects for development.
