ObjectiveThis article proposes to establish the emergency nursing comprehensive case base and carries out workshop in the form of teaching. Then to explore the emergency nursing comprehensive case workshop’s effect of clinical decision making ability on nursing undergraduates. Therefore, this article tries to put forward an effectively activity plan to promote their clinical decision making ability and make an empirical research. Besides, it could also provide some reference significance for college courses relating nursing clinical decision making in the future.MethodsThis research was mainly divided into three stages.The first stage is to establish the comprehensive case base of emergency nursing care.In this phase, researchers mainly use semi-structured interviews, literature review, reading books and magazines, and visiting excellent websites to collect andsort the cases. Then through choosing and discussing repeatedly by researchers,through demonstration by the expert group member, and ultimately establish the comprehensive case base of emergency nursing case.The second stage was to carry out the series of activities in workshop and the intervention study.This stage is quasi-experimental design. Basing on recruitment situation and considering the learning form and the concrete implementation strategy of workshop,sixty one undergraduate nursing students, graded three and from 16 dormitories, were randomly assigned to the experimental group and the control group. Every dormitory as a unit and sorted this 16 dormitories to eight little units based the total number of students in the dormitory numbered 1 to 16. Then two quarters divided a group, such as 1 and 2 forms the first group, 3 and 4 forms the second group and so on. All this dormitories were divided into 8 groups. Then it randomly selected one dormitory in each group to experimental group and the other into the control group, 33 students were in the experimental group and 28 students were in the control group. On the basis of the control group’s intervention program( participate in the normal teaching and the interactive learning network), the experimental group participate in the ten weeks series theme activities of workshop regularly.All students were investigated using general information questionnaire, clinical decision-making ability of undergraduate nursing students questionnaire and general self-efficacy scale(GSES) as baseline and one week after intervention. At the end of the workshop, two groups should participate in the first aid nursing comprehensive experimental, in order to objectively evaluate their practice ability and the ability of response. Data were collected by strict check then input SPSS17.0 statistical software.The statistical methods including descriptive statistics analysis, χ2 test statistics,independent sample t test and paired t test.The third stage: Experience among undergraduate nursing students after participating the comprehensive case of emergency nursing care workshop: a qualitative research.15 members of workshop were selected by using purpose sampling according to the phenomenological method, data were collected by semi-structured interview andanalyzed with seven step analysis of Colaizzi.ResultsThe first stage: the establishing of comprehensive case base of emergency nursing care workshop.The members of the research group preliminary established emergency nursing comprehensive case database, which included 36 cases. Through demonstration by members of the expert group, delete incomplete data of four cases, increase three typical cases, and suggest the case scenarios setting obstacles and accidental,eventually accepted a total of 35 cases into the comprehensive case base of emergency nursing care.The second stage: carry out of the theme series of workshop and control study.The two groups of nursing students were compared with baseline, the difference was not statistically significant(P>0.05).One week after the workshop, it used the independent sample t test and paired t test to analysis the score of experimental group’s clinical decision making ability and general self-efficacy scale(GSES) and they were significantly higher than baseline(P<0.001) and those in the control group(P<0.001). Besides, in the comprehensive design experiment of first aid nursing, the score of experimental group was significantly higher than the control group(P<0.001).The experimental group students were satisfied with the workshop. Using the hermeneutic phenomenology method of qualitative research, the reflective diaries of 33 members were repeatedly read, and analyzed with category approach. Two themes and seven sub-themes were extracted:(1)active thinking mode: improve thinking ability, actively decision making,build clinical situation, elimination of cognitive errors.(2)enhance role identity:improve professional identity, enhance self-value, cohesion team strength.The third stage: Experience among undergraduate nursing students after participating the comprehensive case of emergency nursing care workshop: a qualitative research.The nursing students have better clinical performance. Through the extraction of the theme, two themes and 8 sub-themes were extracted:(1)Clinical decision-making ability is improved: positive thinking and connect action, active communication and collect information of patients, comprehensive and meticulous to improve the observation, reasonable inference and evaluation and judgment.(2)Hope for the course of clinical decision-making: curriculum nature, the teaching method, the course content and the course assessment.ConclusionComprehensive case of emergency nursing care workshop is a feasible,effective teaching organization form, it could strengthen undergraduate students’ clinical decision-making ability and self efficacy. Besides, it also could improve their respond ability in the first aid nursing comprehensive experiments. Therefore, the workshop has diversity teaching methods and the advantage of "teaching- practice-study" trinity of education mode, it is worth to be carried out in other colleges and universities.
