Objective: Compare the direct expenditure of peritoneal dialysis and hemodialysis for ESRD patients. Analyze the difference of disease direct expenditure between peritoneal dialysis and hemodialysis, explore the influencing factors of patients’ choice, recommend treatment with less direct expenditure, and provide empirical basis for health administrative departments and medical insurance agencies to make health policies. Meet the demand of ESRD patients’ treatment, reduce the burden of medical costs, and save social health resources.Methods A questionnaire survey was conducted in 10 pilot hospitals in Henan province. All the patients with renal dialysis in these hospitals were included in the study. We conducted questionnaire survey in their process of diagnosis and treatment, and reviewed their annual disease expenditure from medical record data.Results:(1) There was no significant difference in general demographic information between patients with peritoneal dialysis and hemodialysis(P>0.05).(2) There wassignificant difference in the type health insurancebetween patients with peritoneal dialysis and hemodialysis(P<0.05).(3)There were significant difference in direct medical expenditure between patients with peritoneal dialysis and hemodialysis(P<0.05). The disease direct expenditure of patients with hemodialysis was far more than those with peritoneal dialysis.(4) The personnal and family income of patients with hemodialysis were more than those with peritoneal dialysis before and after dialysis(P<0.05).(5) There was no significant difference in the effect of treatment between patients with peritoneal dialysis and hemodialysis.(P>0.05)(6) The treatment satisfaction of patients with peritoneal dialysis was higher than those with hemodialysis(43.43%, 25.91%, respectively, P<0.05.)(7) There was no significant difference in primary etiology between patients with peritoneal dialysis and hemodialysis. And the direct expenditure of each primary etiology was consistent with the overall, which proved that primary etiology is not the reason for the difference of direct expenditure.Conclusions: The direct expenditure of patients with hemodialysis was higher than those with peritoneal dialysis, there was no significant difference in the effect of treatment between patients with peritoneal dialysis and hemodialysis, but peritoneal dialysis is more convenient than hemodialysis. the hospital guide and health policy has affected the choice of dialysis patients, strengthen primary peritoneal dialysis room construction,guide patients choice economic and reasonable way of dialysis.
