Operative Treatment Of An Acute Displaced Acetabular Fracture In An Older Population

Objective The purpose of the present study was to examine the safety and efficacy of operative treatment of an acute displaced acetabular fracture in an older population.Methods Between October,1991, and March, 2015, In this single-surgeon, retrospectively maintained database, 77 patients were older than 60 years of age at the time of initial surgery and had an average age of 71 years(range; 60–90), with 61 male and 16 female participants.Pre-operative roentgenograms including anterior – posterior and Judet views were obtained along with computed tomography(CT+3D reconstruction) to assess fracture pattern. Surgical management of this older series were made according to an established indication. The time from injury to surgery ranged from 3-21 days,with a mean of 9 days.Results Average operative time was 90 minutes(range,78–120minutes).Average blood transfusion was 800 milliliters(range,600-1000 ml). Of the total of 77 patients, 61 were followed for at least 1 year.Of the 61 patients, fracture reduction was examined with postoperative X-ray film and showed fracture reduction was anatomic(displacement<1mm) in 36cases(59%),imperfect(displacement<3mm) in 25 cases(41%). All patients were found to have healed their acetabular fractures at an average of 3 months(range, 2–4 months).No patient suffered from complication such as displacement,nonunion,deformity,hip dislocation and necrosis of femoral head,except 3 patients suffered from mild narrow of the joint space. According to the modified Merle d′Aubigné and Postel evaluation standard,58 cases(95%) were excellent and good, 3cases(5%) were fair.Conclusion Operative treatment for an acute displaced acetabular fracture in an older population was the safety and efficacy.
