Objective:To investigate the risk of pulmonary metastases from differentiated thyroid carcinoma factors; curative effect evaluation of 131I in the treatment of lung metastases of differentiated thyroid carcinoma.Methods:a retrospective analysis of 145 cases from March 2013 to August 2015 in the First Affiliated Hospital of Guangxi Medical University Department of nuclear medicine in the treatment of patients with differentiated thyroid carcinoma, among lung metastasis group 45 cases, non lung metastasis group 34 cases, with the cinical data of patiens, sex,age, course, surgical procedure, postoperative pathology, focus scope,the lymph node dissection and lymph node metastasis; Among 35 cases of this patients with pulmonary metastases,the whole body scan with 131I suggest that the lesions disappeared or reduced, and (or) CT/X showed pulmonary metastases disappeared or decreased, and (or) the decrease of Tg level as an index to evaluate the efficacy of treatment with131I. X2 test was used for univariate analysis and multivariate analysis was performed using Logistic analysis to investigate the risk factors of pulmonary metastasis and curative effect evaluation of 131I in the treatment of differentiated thyroid carcinoma with lung metastases.Results:a total of 145 patients with differentiated thyroid carcinoma,39 cases of male,106 cases of female, age (37.82±15.68) ears; 45 cases of lung metastasis, 100 cases of non lung metastasis; single factor analysis showed that age (X2=7.70, P=0.03), course (X2=13.57, P=0.00), surgery (X2=8.05, P=0.01), focus scope (X2=10.16, P=0.00), lymph node metastasis (X2=9.21, P=0.01) is associated with lung metastasis; multivariate analysis showed that age, course, surgical procedure, focus scope, lymph node metastasis is an independent risk factor for lung metastasis, Logistic (P)=-1.58-0.83X1 (age) +0.88X2(course)-1.28X3 (surgical procedure)+1.54X4(focus scope)+1.36X5 (lymph node metastasis); 35 cases of patients with curative effect to follow-up, 5 cases were complete cured,26 cases improved,4 cases were invalid, the complete cure rate was 14.29%(5/35), effective rate was 88.57%(31/35), inefficiency rate was 11.43%(4/35).Conclusion:age, course, unilateral thyroid resection, bilateral lesions, lymph node metastasis is the risk of differentiated thyroid carcinoma with lung metastases;131I is an effective therapy for patients with lung metastases of differentiated thyroid carcinoma.
