Investigate The Efficacy Between Radiofrequncy Ablation(RFA) And Surgical Resection In The Treatment Of Small Recurrent Hepatocellular Carcinoma (HCC):an Retrospective Analysis Of 57 Cases

Abstract/Summary:
Objective To investigate the efficacy between radiofrequency ablation (RFA) and surgical resection in the treatment of small recurrent hepatocellular carcinoma (HCC).Methods A search of the patients with small recurrent HCC (tumor size of 3cm or less and no more than 2 nodules), the First Affiliated Hospital of Guangxi Medical University from December 2012 to June 2014. The patients treated by surgical resection (52.6%) and RFA (47.4%), retrospective observational study of the clinical efficacy and recurrence of 57 patients with small recurrent HCC. The measurement data were analyzed by chi-square test. Kaplan-Meier method was used to evaluate the overall survival. The log-rank test was used for disease-free survival (DFS) difference analysis. The Cox regression analysis was used for determining the risk factors for recurrence.Results The 1-.2-.3-years overall survival rates were 96.7%,93.3%, 93.3%and 96.3%,88.5%,84.0% in RFA group and surgical repeated resection group, respectively. There was no significant difference between two groups (P > 0.05). However, the disease free survival was better in repeated surgical resection than in RFA, which were 73.3%,70.0%,64.3% and 46.7%,26.7%, 20.0% in repeated surgical resection and RFA, respectively (P< 0.05). The interval between recurrence and initial hepatectomy with more than 2 years, and the AFP<20ug/L of recurrent HCC were independent factors favor to good prognosis.Conclusions The treatment of RFA for small recurrent HCC were safe, effective and lower complication. The disease free survival in RFA group was lower than surgical resection group, but overall survival was no significantly difference in RFA group as compare with surgical resection group. RFA may be a important therapy of small recurrent hepatocellular carcinoma.
