Objective:To comparison the clinical characteristics and serum TK1, CA125, HE4 levels between borderline ovarian tumors and epithelial ovarian carcinima, Differential diagnosis of diseases before assisted surgeryMethods:Collected in october 2014 to february of 2015 in tumor hospital of guangxi medical gynecologic oncology, A total of 67 cases of Epithelial ovarian cancer (Epithelial ovarian carcinima, EOC group) and 38 cases of ovarian tumors (Borderline ovarian tumors, BOTs group), which were confirmed by pathology. Using retrospective analysis to analyze the clinical data of two groups of patients were retrospectively analyzed, compared two groups of patients with general condition, tumor markers (TK1, CA125, HE4) level, surgical procedure and postoperative pathology.Results:(1) BOTs group of patients with an average age was (39.94+ 16.70), EOC group average age was (51.75+10.58), the difference was statistically significant, in the patients of premenopausal, BOTs group of patients with an average age was (38.28+7.79), lower than the EOC group (47.76+4.71), the difference was statistically significant. (2) The occurrence probabilities of abdominal distension, abdominal pain, compression symptoms, in EOC patients was higher than that of BOTs group, the difference was statistically significant; (3) The elevated ratio of HE4, EOC compared BOTs patient is higher, the patients of TK1, CA125, HE4 were increased, elevated in patients with EOC were significantly higher than the BOTs; (4) EOC patients in 98.51% to be a comprehensive staging surgery, BOTs group of patients,57.89% were unilateral oophorectomy or ovarian tumor resection,28.95% were surgical staging; (5) BOTs in the group of patients,27 cases (71.50%) were mucinous tumors,11 cases (28.95%) were the serous tumors, did not see the other types. EOC group of patients,38 cases (56.72%) were the serous tumors,20 cases (29.85%) were mucinous tumors,3 cases (4.48%) were endometrial samples,6 cases (8.96%) were clear cell,3 cases (4.48%) were mixed. Two groups of patients with tumor pathological type distribution difference was statistically significant; (6) In the group of patients BOTs, the levels of HE4 in Late (III phase) were significantly higher than early (I-II stage). In the group of patients BOTs, the levels of CA125 and HE4 in Late (III-IV stage) were significantly higher than early (I-II stage). (7) In the group of patients BOTs, the difference levels of serum TK1, CA125, HE4 in the concentration serous tumors and other types of tumors was no statistical significance. In the group of patients BOTs, the levels of CA125 and HE4 in the serous tumors were higher than other types of ovarian cancer (mucinous, endometrioid, clear cell, mixed), the difference was statistically significant.Conclusion:(1) BOTs occurs in women of reproductive years, EOC occurs in perimenopausal women; in the postmenopausal patients, The prevalence of age in BOTs was younger than EOC. (2) The level of tumor markers of TK1、 HE4 and CA125 of the patients, EOC patients than in patients with BOTs increased to a large extent.(3) The mainly common pathological type of BOTs is serous, mucous sex; The mainly common pathological type of EOC is serous, mucus, endometrial, clear cell, mixed, other types; Sexual serous epithelial ovarian cancer CA125, HE4 concentration higher than that of other types of ovarian cancer, can assist to judge pathological types. (4) In age, menopausal status, histological type, tumor markers elevated situation aspects, BOTs patients with EOC patients were different. (5) On the treatment, BOTs use of unilateral oophorectomy or ovarian tumor resection, EOC use of ovarian cytoreductive surgery.
