Effect Research Of The Family Management Intervention On 0 To 6 Years-old Epileptic Children In A Hospital

Objective: Evaluate the effect of family management intervention on family function of epileptic children aged 0 to 6 years old、parents’ ability of disease management、parents’ disease knowledge、parents’ life quality and drug adherence、epilepsy frequency of attack、each time of duration.Methods: Belongs to the types of experimental research, this study randomly selected 106 family with epileptic children aged 0 to 6 years old from a third class A children’s hospital on June 1, 2015 to November 30 as the research object, The epileptic family from the neurology ward one set as the control group, the other family from the neurology ward two as intervention group,the control group and the intervention group both have 53 families.Diagnosed with epilepsy within 24 hours,researchers issued questionnaires and scale to children of two groups(children and their families general condition questionnaire,family function evaluation form,family management scale- disease management difficulties, two scale from disease management ability, epileptic disease knowledge questionnaire, the quality of life measurement scale)to survey the baseline condition,and determinate the serum concentration of antiepileptic drugs by patients with venous blood sampling to test the patient’s medication adherence.Then the control group accepted routine treatment and nursing and health education;Except the conventional treatment, nursing and health education, the intervention group have accepted the family management intervention by researchers, mainly divided into two parts,epilepsy disease knowledge and management ability, family functions and tasks, the intervention time is divided into hospitalization and after discharge, intervention approach including teaching, distributed data, individual guidance and instruction demonstration, watch video, companion to share, discuss communication and telephone urged, etc. Both groups was investigated again in 3 months back to court to review after discharge through the questionnaire and scale, to evaluate the intervention effect on parents’ observation indexes who have the epileptic children,including family function, disease management ability, epileptic disease knowledge and quality of life,and the epileptic children’s observation indexes, including determinate the serum concentration of antiepileptic drugs by patients with venous blood sampling to test the patient’s medication adherence, nearly three months seizure frequency and duration of each attack.Results: This study extracted 106 family that have epileptic children, 92 family have completed the entire research process,the lost rate was 13.21%, the control group have lost eight family, the lost rate was 15.09%; The intervention group have lost 6 family, the lost rate was 11.32%,the lost situation of two groups has no statistically significant difference(p> 0.05). This study results show that:(1) the comparison between the two groups before intervention:parents’ observation index of two groups(family function, disease management ability, epileptic disease knowledge level and quality of life)has no statistically significant difference(p> 0.05);Compared the observation index the observation index from two groups(the blood concentration of antiepileptic drugs to test medication adherence in children with epilepsy, nearly three months, each attack duration),there are no statistically significant difference(p> 0.05).(2) the comparison between the two groups after intervention: parents’ observation index of two groups(family function, disease management ability, epileptic disease knowledge level and quality of life) has statistically significant difference after statistics analysis(p< 0.05);The patient’s medication adherence test by the blood concentration of antiepileptic drugs, after statistics analysis, the difference was statistically significant(p< 0.05); But the seizure frequency for nearly three months, every time duration and so on have no statistically significant difference after statistical analysis(p> 0.05).(3)The comparison of the control group before and after the study: parents’ observation index of the control group(family function, disease management ability, epileptic disease knowledge level and quality of life), and the patient’s observation index(blood concentrations of antiepileptic drugs to test the patient’s medication adherence, nearly three months seizure frequency and duration of each attack) have change, but changed little, after statistics analysis, there have no statistically significant difference(p> 0.05).(4)The comparison of the intervention group before and after the study:parents’ observation index of intervention group(family function, disease management ability, epileptic disease knowledge level and quality of life) are better than before(p< 0.05); Compared the patient’s blood drug concentration of antiepileptic drug which used to test patients’ medication adherence,there have statistically significant difference after statistics analysis(p< 0.05); the comparison of nearly three months of epileptic seizure frequency and duration of each attack have changed, but changed little, after statistics analysis,the difference have no statistically significant(p> 0.05).Conclusion:(1) The family management intervention can improve the family function of epileptic children aged 0 to 6 years old,and the level of parent knowledge of epileptic disease, disease management and quality of life.(2)The family management intervention can improve the medication adherence of 0 to 6 years-old epileptic children,and maintain the normal blood concentrations of antiepileptic drugs.but it has not proved that the family management intervention can make a difference on seizure frequency for nearly three months and the duration of each attack of epileptic children.
