Part Ⅰ Application of 3D-ASL in Study of Transient Ischemic Attackobjective To investigate the value of three dimensional arterial spin labeling (3D-ASL) in the patients with TIA.Materials and Methods 27 cases of patients were enrolled in the study within 24 hours of symptom onset.27 cases undergo CT DWI MRA and 3D-ASL. The definition of region of interest (ROI) was that when the relative cerebral blood flow (rCBF) ratio less than 0.8 between the ROI and the mirror area, two observers hand drawn ROI and the mirror image area alone at the maximum level of abnormal perfusion in the pseudocolor images, and quantitative analysis the rCBF.Results CT had negative results. Qualitative analysis showed that 27 patients with 3D-ASL were found in 23(85.2%) with focal hypoperfusion, quantitative analysis showed that the rCBF of ROI was 28.98±8.12 ml/100g·min, the mirror image area rCBF was 40.58±9.15 ml/100g·min, the rCBF between ROI and the mirror region was statistically significant (t=-9.59, P< 0.05). Overall,14 of 27 patients (51.9%) were MRA-positive and all of the 14 cases were ASL-positive, 13 patients (51.9%) were MRA-negative and 9 cases ASL-positive; 10 of 27 patients (51.9%) were DWI-positive and 9 cases were ASL-positive,17 patients (51.9%) were DWI-negative and 14 cases ASL-positive.3 patients after treatment showed recovery of the focal hypoperfusion.Conclusion Compared with the MRA and DWI,3D-ASL showe more sensitive and direct of focal hypoperfusion in TIA, and has important value in patients with TIA diagnosis, curative effect evaluation and follow-up.Part Ⅱ Comparative Study of 3D-ASL and DSC-PWI in Transient Cerebral AttackObjective To evaluate the application value of 3D-ASL compared with DSC-PWI in TIA.Materials and Methods 5 cases of patients were enrolled in the study within 24 hours of symptom onset.5 cases undergo CT DWIMRA 3D-ASL and DSC-PWI. The definition of region of interest (ROI) was that when the relative cerebral blood flow (rCBF) ratio less than 0.8 between the ROI and the mirror area, two observers hand drawn ROI and the mirror image area alone at the maximum level of abnormal perfusion in the pseudocolor images, and quantitative analysis the rCBF.Results Qualitative analysis showed that 5 cases (100%) of the 2 kinds of perfusion imaging technology were consistent with the low perfusion. Quantitative analysis showed that the rCBF ratio between the ROI and the mirror area of 2 kinds of perfusion imaging technology, ASL was 0.52±0.17, DSC was 0.56±0.19, and there was no significant difference between the two (t=0.388, P>0.05).Conclusion 3D-ASL showe similar sensitivity with DSC-PWI,and have important value in patients with TIA diagnosis, curative effect evaluation and follow-up.
