| Purpose:This study was based on the clinical follow-up study on the treatment of patients with giant lumbar disc herniation by elimination of nuclear marrow soup, and to observe the therapeutic effect of LDH, and by comparing with the treatment of intervertebral foramen, analysis of the advantages and disadvantages of the conservative treatment and the intervertebral foramen treatment of lumbar disc herniation, aims to better guide clinical.Methods:LDH patients who were admitted to the Chinese Medicine Hospital of Suzhou city from March 2015 to June and completed the follow-up of -2016 patients. According to the imaging diagnosis of giant LDH patients in 80 cases, eliminating the elimination of nuclear marrow soup and foraminal mirror as the main means of treatment, efficacy evaluation was performed using JOA score, ODI index, Straight leg raising test and finger ground distance; through the observation of MRI, the absorption rate of giant HNP was studied, and the correlation between the length of disease and the resorption was studied.Results:(1) Treatment of huge lumbar disc herniation and lumbar intervertebral disc herniation with the combination of elimination of nuclear marrow soup and Western medicine can improve the low back pain JOA score and the index of the ODI, the intervertebral foramen In the 3 month (p<0.01) and 6 months (p<0.05) compared with the conservative treatment group, the effect is better than the traditional Chinese medicine conservative treatment group. But from the long-term effect of the point of view (6 months-1 years), the two groups of curative effect is similar. (p>0.05) (2) The improvement of straight leg raising test after the operation of the lumbar intervertebral foramen was obvious, which showed that the root of the patients were relieved significantly after the removal of the protrusion, from the long-term efficacy of the two, there is no statistically significant difference between the two, it is also explained that the root cause of the patient is not only caused by mechanical compression, but also can be caused by inflammation and edema. (3) In return of 36patients by the prominent rate and absorption rate of Statistics, found that in tire soup nucleus pulposus of conservative treatment group,5 cases appeared incompletely absorbed (absorption rate is more than or equal to 75%),5 cases of complete absorption, this situation accounts for 25% of the conservative group. The author thinks that the elimination of the nucleus pulposus soup can make the probability of the absorption of the huge lumbar disc herniation is greatly improved. And according to the course of disease and the absorption rate of the study, found that the shorter duration of huge protrusion of lumbar intervertebral disc, the higher the probability of HNP appears to re absorption.Conclusions:(1)The therapeutic effect of elimination of nuclear marrow soup on the huge lumbar disc herniation is better, and can promote the re absorption of HNP. (2) The shorter the course of patients with giant LDH, the more prone to HNP reabsorption. (3) Compared with traditional Chinese medicine, the treatment of hole mirror has obvious curative effect, the underlying symptoms were significantly relieved, but no significant difference in long-term curative effect, so it should be strictly grasp the operation indication to the doctor. |
