| Objective:Retrospecting single tooth soft tissue compare the recovery condition with pre-operation, wearing and regular visits of 28 patients with immediate implants in maxillary anterior. Applying the appropriate aesthetics indicators to assess and analysis study soft tissue recovery in maxillary anterior. Esthetics covery of soft tissue in maxillary anterior for clinical immediate implant restoration to provide some referenceMethod:In accordance with the inclusion criteria were selected to accept the conditions of 28 patients with single immediate implants maxillary anterior. All patients has implant surgery with GBR techniques, while no increase of soft tissue. Patients aged between 21 to 60 years, mean age 39.89±11.17. Male 13 persons is accounting for 46.43%, female 15 persons is accounting for 53.57%. Were recorded soft and hard tissue measured values of immediate implants for each patient pre-operation, operation,wearing and regular visits. Through the measurements in oral, photos records, periapical radiography and CBCT imaging data observed assess change of immediate implant with the position and shape of gingival margin and gingival papilla, width of attached gingiva, peri-implant bone and the PES after wearing.Part I:Comparison with the measured data of soft tissue for immediate implant prosthodontics patients in anterior. Part II:According to the preoperative or operative evaluation(implanting causes,defect bone type, gingival biotype, implant systems, implant models etc.) of different patients to assess the change of soft tissue after immediate implant in maxillary anterior. Count data using the percentage of representation, measurement data were expressed as mean±standard deviation, by χ2 test, analysis of variance, repeated measures analysis of variance statistical methods of data analysis and processing.Results:1, Morphology symmetrical rates of gingival margin is with immediate wearing was 50%,wearing 1 to 6 months was 57.14% wearing 1 year was 66.67% and wearing 2 years was 72.22%. Evaluation with different time changes of gingival margin without statistical significance (p<0.05).2, the width of the attached gingiva is with immediate wearing was 5.12±0.72mm, wearing 1 to 6 months was 5.17± 0.74mm, wearing 1 year was 5.23±0.67mm and wearing 2 years was 5.28±0.53mm, Evaluation with different time changes of attached gingival width without statistical significance (p<0.05).3, papilla index of 3 percent is in immediate wearing was 28.57%, wearing 1-6 months was 50%, wearing 1 year was 85.71% and wearing 2 years was 83.33%. Evaluation with different time changes of papilla index (x2= 22.542, p value<0.001) were statistically significant.4, PES score is with immediate wearing was 10.14±1.69, wearing 1 to 6 months was 11.29±1.49, wearing 1 year was 11.55±1.23 and wearing teeth 2 years 11.72±1.02, Evaluation with different time changes of PES (F=12.785, p<0.001) were statistically significant, PES rates increase over time increased wearing.Part Ⅱ:the soft gum tissue and preoperative evaluation index factors (reasons for missing teeth, bone defect type, gingival biotype, gingival papilla position) correlation.1, the gingival margin morphology changes and intermediate gingiva biotypes (x2=5.939, p<0.05) was statistically significant; 2 changes, the width of the attached gingiva and preoperative evaluation showed no statistical significance (p<0.05); 3, change papilla index and fixed partial denture failed (x2= 15.479, p<0.001), type of B+C bone defect (x2= 21.917, p<0.001), intermediate gingival biotypes (x2= 19.949, p<0.001), preoperative gingival recession nipple position (χ2= 17.611, p <0.001) 4 preoperative evaluation index was statistically significant; 4, PES score changes and reasons for missing teeth, gingival biotypes, preoperative assessment papilla situation was no interaction (p> 0.05), PES score changes and different types of bone defects (F= 17.732, p= 0.001) linear trend.Conclusion:1, Evaluation four index of gingival soft tissue (gingival margin morphology, gingival papilla index, PES) in the wearing immediately to wear teeth two years periodic follow-up visits is a certain recovery with the sufficient and stable alveolar bone. But width of attached gingiva with growth of time has little change in observation period.1) Assessment at different time, the gingival margin morphology of symmetry versus time for no apparent close relationship. But in the immediate tooth wear to wear a two-year observation period after the teeth, gingival margin morphology symmetric rate increases with time there is a certain increase.2) Assessment at different time, little change in the width of the attached gingiva.3) Gingival papilla index increased with increasing wearing time, after wearing the most significant change wearing 1 year.4) Immediate wearing to wearing 1 year PES scores changed significantly, it has a growing trend.But after wearing 1 year PES slowing growth rates stabilized2, After immediately implant placement, gingival soft tissue Indexes and preoperative evaluation index (missing teeth cause bone defect type, gingival biotype, gingival nipple position) the correlation analysis the following four points.1) Whether the gingival margin morphology symmetric and intermediate gingiva biotypes are closely related.2) Width of attached gingiva preoperative evaluation showed no change and close relationship.3) Gingival papilla index changes in fixed partial denture failure of edentulous reason, B+C type bone defects, intermediate gingival biotypes, preoperative retraction gingival position is closely related cases.4) Different evaluation time with PES score changes and different implanting causes,gingival biotypes and preoperative evaluation papilla situation may not closely related. But PES tooth wear over time the average score increases, different evaluation time of PES rates and different bone defect type may be closely related, PES tooth wear rates over time increases, and the linear correlation... |
