Objective(S):Through Mini-mental State Examination (MMSE)and Montreal Cognitive Assessment Scale (MoCA) to evaluate patients’Cognitive function, screening and collecting patients with mild cognitive impairment, and further to take a diffusion tensor imaging Examination(DTI)of magnetic resonance, In order to provide the basis for early clinical diagnosis of mild cognitive impairment we study the relationship between mild cognitive impairment and the diffusion tensor imaging of white matter,Analysis patients with mild cognitive impairment neuropsychological scale for evaluating the correlation of results with DTI findings.Methods:All cases are form the third medical department of leading carders in the Second Hospital of Kunming Medical University, which are under treatment in the hospital from August 2015 to January 2016。Recording the patients general condition, such as gender, age, educationanl years; previous history is also needed, such as history of stroke, psychosis and then eliminating the cases do not meet the conditions. All the objects are included in the research is to be rated through the Hamilton depression rating scale, daily life ability rating scale and dementia rating scale then recording the score for further screening. Mild cognitive impairment group are included in 34 cases and control group are 22 cases eventually in the research, and all the cases undergoing routine head magnetic resonance imaging and diffusion tensor imaging. The upload original images to the Extended MR WorkSpace 2.6.3.4 workstation of Philips company when the scanning program is completed in the Philips Achieva 3.0 TXS,and reconstruc the smooth image of FA for better tracking the fiber and measurement 。 Using circular rigion of interest (ROI,size is about 20mm2)to measure FA value in different region of brain white matter fiber in FA map of T2 WI EPI (b= 0). In scanning level completely matching routine T1WI and T2WI sequence as anatomical reference images. Each ROI measurement of fiber bundles are using FA color map and T1WI superposition technology tracking with fiber beam in its continuous level of upper and lower level value, arithmetic mean of the three values obtained for the final result. The APP of SPSS 17.0 statistical software is applied to all data for statistical analysis.Results:Comparative analysis between mild cognitive impairment group and control group in the overall situation found no statistical differences among gender, age and the educational years (P> 0.05). The score of MMSE and MoCA in mild cognitive impairment group are lower than the control group, two groups are statistically significant (P< 0.01). And mild cognitive impairment group HDRS score is higher than the normal control group, the difference is statistically significant (p< 0.05). In terms of the cognitive domain scores, mild cognitive impairment group MMSE scores are lower than the normal control group, the cognitive domain of orientation,attention and calculation and copying graphics statistically difference (P < 0.05), and memory ability has significant difference (P< 0.01). Various cognitive domain score of MoCA in mild cognitive impairment group are lower than normal control group, except for the visual space and execution ability and naming ability are no statistical difference, the attention and computing ability, language ability, abstract thinking, memory and orientation ability have statistical significance (P< 0.05), Memory ability, orientation ability and attention and computing ability have significant statistical difference (P< 0.01). Patients with mild cognitive impairment group compared with normal control group found the FA value of the white matter of left frontal lobe, parietal lobe,right temporal lobe,occipital lobe and the corpus callosum were no statistical difference (p> 0.05). the FA value of white matter of Right frontal lobe (0.335 ± 0.068), left temporal lobe (0.391 ± 0.032) and the corpus callosum knee (0.658 ± 0.053) in the MCI group are lower than the corresponding parts in the NC group which have statistical significance (p< 0.05); the FA values and the cognitive domain of MoCA scale which both had statistically significant by comparing MCI group and NC group using canonical correlation analysis,. found that R value between the FA value of white mater of right frontal lobe and attention and calculative ability was 0.332, between the FA value of left temporal lobe and memory was 0.456, between the FA value of orpus callosum knee and memory was 0.429, all p values were less than 0.05 with statistical significance, the FA values and the cognitive domain of MMSE scale which both had statistically significant by comparing MCI group and NC group using canonical correlation analysis, found that only the R value between the FA value of white mater of right frontal lobe and attention and calculative ability was 0.342, P value is less than 0.05, with statistical significance。Conclusion(s):Patients with mild cognitive impairment may be more easily with depressive disorder than normal cognitive function people. Patients with mild cognitive impairment of memory disorders may be related to the white matter damage of left temporal lobe and the corpus callosum knee, attention and calculation ability disorder may be associated with the white matter damage of right frontal lobe. Brain white matter lesions can lead to cognitive dysfunction, magnetic resonance diffusion tensor imaging can provide objective basis for early diagnosis of MCI which show decreased FA values of the corresponding damage brain white matter.
