| Objective:To observe the clinical characteristics of 61 prolactinoma patients, analyse the surgical results,and discuss the best treatment of prolactinomas in patients.Methods:A total of 61 patients with PRL adenoma underwent surgery at the Department of Neurosurgery, First Affiliated Hospital of Kumming Medical University from Sep 2012 to Apr 2016 were enrolled.To observe the common symptoms of prolactinoma in patients; To analyse the relationship between the menstrual changes and serum PRL level; To analyse surgical results of macroadenoma and microadenoma;Results:1. The most common symptoms of prolactinoma in female patients were menstrual changes and lactation; The most common symptoms of prolactinoma in male patients were headache and diminution of vision. There existed no statistical significance of tumor size between male and female patients 2 There existed statistical significance between the clinical features of the serum PRL higher than 200 ng/ml and the serum PRL lower than 200 ng/ml (x2=6.810, P=0.009<0.05).3. The cure rate of microadenoma was 84.21%,the cure rate of macroadenoma was 57.14%;The resection extent of microadenoma was higher than that of macroadenoma (z=-3.888, p=0.000<0.05),and the curative effect of microadenoma was better (x2=4.246, p=0.039<0.5).Conclusion:1.Female symptoms were often caused by endocrine disorders, so they went to see a doctor earlier,and tend to choose DAs as initiate treatment. When they choose surgery,the volume of the tumor was big. Male symptoms were often caused by tumor mass effect, so they went to see a doctor later,at the moment,the size of male patients was big.2. When the serum PRL was higher than 200 ng/ml, the menstrual change was much more serious, amenorrhea was more likely to be caused.3. Whether it was microadenoma or not, DAs are the gold standard treatment of prolactinoma.With the development of minimally invasive surgery, the curative effect was much higher than before, especially in the microadenoma, therefore, surgery could be the first choice in some patients. |
