A Study Of Correlation Between Left Ventricular Remodeling Index And Gensini Score For Hypertension Patient Accompany With Coronary Heart Disease

Objective:To discuss the correlation between left ventricular mass index and and the Gensini score of coronary heart disease patients with hypertension,to detect the different of left ventricular mass index between hypertension patients and coronary heart disease patients with hypertension in different grade of Gensini score.Thus, provide guidance for patients accept coronary angiography or not.Method:Retrospected 150 hypertension patients accepted coronary angiography and UCG in the First Hospital of Kunming from 2014.3-2015.12.There are 30(16.7%) hypertension patients without coronary heart disease and 120(66.7%) coronary heart disease patients with hypertension.Besides.there are 30 healthy people as mock group.We record the case of illness,data of UCQresults of coronary angiography and results of blood biochemistry.Gensini score was used to assess the coronary heart disease patients with hypertension.Results:There are 180 cases in our study,120(66.7%)of them are male and 60(33.3%) are female.There are 30(16.7%) cases in mock group(group A),hypertension patients(group B) account 30(16.7%) and 120 coronary heart disease patients with hypertension(group C) take the 66.7% of all.We separate the coronary heart disease patients with hypertension into 3 groups based on state of illness.Mild group 30 patients account 33.3%, moderate group 30 patients (33.3%) and severe group 30 patients (33.3%).(1) In case of illness,age,cerebrovascular disease,smoking, systolic pressure,diastolic pressure and BMI were compared between group A,B,C.B and C significantly higher than A (p< 0.05).(2) In the comparation of results of blood biochemistry,TC:B,C>A,HDL-c:B,C<A (p<0.05).LDL-C:A<B<C (p<0.05).(3) LVEF:C<A and B, LVM and LVEDV:C>A and B (p<0.05),LVRI:A>B>C (p <0.05).(4) LVEDV:C1<C2<C3,LVRI:C1>C2>C3 (p<0.05). LVEF:C1>C2 and C3 (p< 0.05).(5) LVM,LVEDV,LVEF and LVRI was considered has high correlation with Gensini score (p<0.05). LVM and LVEDV has positive correlation with Gensini score (r>0). LVEF and LVRI has negative correlation with Gensini score (r< 0).Level of correlation:LVRI>LVEF>LVEDV>LVM (|rLVRI|>|rLVEF|>|iLVEDV|>|rLVM|)Conclusion:1.For those coronary heart disease patients with hypertension,it is demonstrated an significant negative correlation between left ventricular remodeling index and Gensini score.2.1t was observed that left ventricular remodeling index has much closer correlation with Gensini score than left ventricular mass,left ventricular end-diastolic volume and left ventricular ejection fraction.3.Left ventricular remodeling index can be regarded as an prognosis value for hypertension patient accompany with coronary heart disease or not.
